广州市某三级医院老年高血压病合并脑卒中患者自我效能与自觉健康控制的相关分析

摘　　要：目的探讨老年高血压病合并脑卒中患者自我效能与自觉健康控制的水平并对其相关性进行分析。方法采用健康行为自我效能量表及自觉健康控制量表，对广州市某三级医院100例老年高血压病合并脑卒中患者进行调查。结果本研究共调查高血压病合并脑卒中患者103例，回收问卷100份，有效回收率为97．09％。本组患者健康行为自我效能得分总分为（59．75±16．61）分，62％处于中等水平，其各维度平均得分分别为：健康责任（19．25±4．25）分，营养（10．97±4．31）分，心理安适（15．14±6．08）分，运动（14．39±5．59）分；自觉健康控制各维度平均得分为：外部权威人士控制（25．69±6．27）分、内部控制（21．61±6．82）分、外部命运控制（20．65±7．45）分；采用Pearson相关分析对患者自我效能与自觉健康控制的相关性进行分析，结果表明，自我效能总分与内部控制、外部权威人士控制呈正相关，（r分别为0．393，0．259；P〈0．01），与外部命运控制呈负相关（r=-0．265，P〈0．01）。结论应重视老年高血压病或高血压病合并脑卒中患者自觉健康内部控制和外部权威人士控制的宣教，提高其自我效能，预防脑卒中的发生或复发。Objective To describe the status of self-efficacy and health locus of control in elder hypertension with stroke patients so as to analyze their correlation. Methods One hundred hypertension with stroke patients were investigated with self-rated abilities for health practice scale and multidimensional health locus of control scale. Results The mean scores of self-efficacy was （59.75 ± 16.61 ） and 62% patients were in the moderate level. And the dimension scores of self-efficacy were that health responsibility （19.25±4.25 ）, nutrition（ 10.97 ± 4.31 ）, psychological comfortable （ 15.14 ±6.08 ） and sports （ 14.39± 5.59）. The HLC scores of patients were that powerful others HLC （25.69±6.27） ,internal HLC（21.61±6.82）, and chance HLC （20. 65±7. 45 ）. Pearson correlation analysis showed that self-efficacy was positively correlated with powerful others HLC and internal HLC （ r = 0. 393, P 〈 0.01 ）, while it was negatively correlated with chance HLC（r = -0.265,P 〈 0.01 ）. Conclusions It is important to enhance the education of powerful others health locus of control and internal health locus of control so as to improve their self-efficacy and reduce the occurrence of stroke and recurrence.
