亲属活体肾移植供者术后长期生活质量随访调查(附41例报告)

摘　　要：目的调查亲属活体肾移植供者术后长期生活质量变化。方法按照国际通用SF-36健康调查问卷对其术后长期生活质量进行评价,并使用为肾移植供者特制的再移植意愿度问卷,了解供者对供肾的态度及进一步探讨可能影响供者抉择的因素。结果术前与术后生活质量比较差异有统计学意义(P<0.05),术后1个月与术前比较,除躯体疼痛、情感角色、精神健康得分有所上升外,余5项均下降,差异有统计学意义(P<0.05);术后1年躯体疼痛得分与术前比较差异有统计学意义(P<0.05),余各项比较差异无统计学意义(P>0.05);术后5年各测评项得分与术前比较差异均无统计学意义(P>0.05)。后悔供肾组的生活质量得分低于愿再供肾组,大部分指标差异有统计学意义(P<0.05)。结论活体亲属肾移植手术安全可行,供者完善的术前评估与选择以及术后严密长期随访、供肾后健康知识的普及十分重要。Objective To evaluate the long-term quality of life in living and relative kidney donors after kidney transplantation. Methods The short form health survey （SF-36） questionnaire was conducted by 41 donors to investigate their quality of life, and a specially prepared questionnaire for the kidney donors was filled by the donors to learn about their attitudes toward kidney donation and for further evaluating influencing factors of choice. Results The differences of quality of life before and after operation were statistically significant（ P 〈 0. 05 ）, scores of emotional role and emotional health one month after operation were higher than that before operation except that of bodily pain while scores of the other 5 items all significantly lower（P 〈0. 05）. One year after operation the difference in score of bodily pain was statistically significant （P 〉 0. 05 ）. Five years after operation, the differences of scores in all the other items were not statistically significant （ P 〉 0. 05 ）. The average scores of quality of life in repentant donor group were significantly lower than that of acceptant donors in most cases （P 〈 0. 05 ）. Conclusion Renal transplantation operation of living relatives is safe. It is very important for donors to have preoperative evaluation and selection and a long term post operation follow-up and health care knowledge are essential.
