梗阻性慢性前列腺炎伏毒内蕴病机特点与治疗观察

摘　　要：【目的】探讨梗阻性慢性前列腺炎"伏毒内蕴"的主要病机特点,并以此为依据应用前列腺炎梗阻方进行治疗,观察并评价其临床疗效。【方法】选择87例经尿动力学等检查确诊为梗阻性慢性前列腺炎患者,辨证符合瘀阻精道、伏毒内蕴、正气不足。以活血通精、化浊解毒、扶正托毒为主要治法,采用前列腺炎梗阻方(由莪术、路路通、郁金、延胡索、黄柏、虎杖、桔梗、野菊花、五味子、酸枣仁、皂角刺等组成)治疗2个月,观察治疗前后患者慢性前列腺炎症状积分指数、膀胱残余尿量、最大尿流率、最大尿道压的变化情况。【结果】治疗后慢性前列腺炎症状积分指数、膀胱残余尿量、最大尿道压显著降低,最大尿流率显著升高,治疗前后比较差异均有统计学意义(P<0.05)。【结论】基于"伏毒内蕴"的病机特点并采用前列腺炎梗阻方辨证治疗梗阻性慢性前列腺炎疗效确切。Objective To explore the characteristics of pathogenesis of obstructive chronic prostatitis and to observe the therapeutic effect of the therapy based on the theory of internal accumulation of latent toxins. Methods Eighty-seven obstructive chronic prostatitis patients confirmed by urinary kinetics were differentiated into blood- stasis accumulating spermatic tract, internal accumulation of latent toxins, and deficiency of healthy qi. Based on the syndrome differentiation, the recipe for obstructive prostatitis was prescribed, and it was mainly composed of Rhizoma Curcumae, Fructus Liquidambaris, Radix Curcumae, Rhizoma Corydalis, Cortex Phellodendri, Rhizoma Polygoni Cuspidati, Radix Platycodonis, wild Flos Chrysanthemi, Fructus Schisandrae Chinensis, Semen Ziziphi Spinosae, and Spina Gleditsiae. The treatment lasted two months. Before and after treatment, scores of inflammatory symptoms, residual urine volume in the bladder, maximal urethral flow rate, and maximal urethral pressure were observed. Results After treatment, scores of inflammatory symptoms, residual urine volume in the bladder and maximal urethral pressure were decreased, and maximal urethral flow rate was increased （ P 〈 0.05 compared with those before treatment）. Conclusion Syndrome differentiation and treatment based on the theory of internal accumulation of latent toxins is effective for the treatment of obstructive chronic prostatitis.
