摘　　要：为明确延续性护理的概念以及了解国外延续性护理模式的研究进展,以促进我国的延续性护理研究,通过在英国、美国等国家卫生网站以及Pubmed等数据库检索延续性护理模式相关报告及文献,发现虽然各研究者对延续性护理的定义不同,但是都强调了卫生服务的"协调、连接和一致"。目前发展较为成熟的延续性护理模式有引导式护理模式、评估和照顾长者的老年资源模式、延续性护理干预模式及APN延续性护理模式。In order to clarify the concept of continuing care and the progress foreign countries had made on the model of continuing care, we searched the reports from the government health website and literatures from Pubmed database. Though researchers define continuing care differently, all of their concepts contain the idea of ＂coordination, connection and consistency＂. Guided Care model, Geriatric Resources for Assessment and Care of Elders model, Care Transitions Intervention model and APN transitional care model are tested models proven to be effective in improving the health results and reducing total health cost.
