摘  要:[目的]观察中药狼疮定对外周血淋巴细胞协同刺激分子表达的影响,探讨其治疗狼疮性肾炎(lupus nephritis,LN)的作用机制。[方法]48只MRL/lpr小鼠随机分成4组,每组12只,分别为模型组、中药(狼疮定)组、泼尼松组、来氟米特组。观察各组间灌胃饲养前后体重变化、尿蛋白水平及存活率,用药16周后抽血测PBLC CD4+CD28+、CD4+CD40L+、CD4+CTLA-4+、CD19+CD80+、CD19+CD86+、CD19+CD40+细胞百分率和肾组织病理变化。[结果]治疗16周后,模型组尿蛋白均高于各治疗组,中药组与泼尼松组无明显差别;泼尼松组和来氟米特组脾脏指数均小于中药组;模型组及中药组CD4+CD40L+细胞及CD4+CTLA-4+细胞均高于泼尼松组和来氟米特组,中药组与模型组CD4+CD40L+细胞差异显著,模型组CD19+CD80+及CD19+CD86+细胞均显著高于其它各治疗组,中药组的CD19+CD80+和CD19+CD86+细胞均高于泼尼松组和来氟米特组;模型组MRL/lpr小鼠肾病理损害明显重于其它各组。[结论]中药狼疮定能抑制MRL/lpr小鼠外周血淋巴细胞CD40L、CD80、CD86表达,对CD4+CTLA-4+抑制作用不明显,因而能平衡淋巴系统致命的过刺激反应,对LN有较好的治疗作用。[Objective ] To observe the effect of Langchuangding on co-stimulating molecules of PBLC in lupus nephritis (LN) to explore the mechanism of Langchuangding in treating lupus nephritis. [Methods ] 48 MRL/Ipr mice were randomly divided into model group, Chinese Medicine group, pred- nisone group and leflunomide group. In the experiment, body weight, urine protein and survival rate were observed. 16 weeks later, anticoagulated blood with bicolor fluorescent labeling were measured to detect the CD4^+CD28^+ cell, CD4^+CD40L^+ cell, CD4^+CTLA-4^+ cell, CD19^+CD80^+ cell, CD19^+CD86^+ cell and CD19^+CD40^+ cell by flow cytometry (FCM). Meanwhile, thymus gland index and spleen index and pathological change of nephridial tissue were detected. [Results ] After 16-week treatment, the level of urine protein in model group was more than other groups. The level of urine protein between Chinese Medicine group and prednisone group had no obvious difference. Spleen index of leflunomide group and prednisone group were obviously smaller than that of Chinese Medicine group. The proportions of CD4+CD40L^+cell and CD4^+CTLA-4^+cell in model group and Chinese Medicine group were more than that ofprednisone group and leflunomide group. Between Chinese Medicine group and model group, the proportion of CD4^+CD40L^+ceU had obvious difference, The proportions of CD19^+CD80^+ cell and CD19^+ CD86^+ cell in model group were obviously more than that in other groups. The proportions of CD19^+CD80^+ cell and CD19^+CD86^+ cell in Chinese Medicine group were obviously more than that in prednisone group and leflunomide group. Pathological changes of MRL/Ipr mice nephridial tissue in model group were worse than other groups. [Con- clusion] Langchuangding can depress the expressions of CD40L, CD80 and CD86, and have better treatment for LN.
