摘 要:目的探讨超声造影评价低温条件下兔肾皮质血流灌注变化的应用价值。方法将20只新西兰大白兔随机分为5组,每组4只:第1组为对照组,第2组-15℃~-23℃低温处理8h,第3组-15℃~-23℃低温处理4h,第4组-6℃~-14℃低温处理8h,第5组-6℃~-14℃低温处理4h。经耳缘静脉团注SonoVue(0.1ml/kg)进行肾灌注造影检查,观察并采集注射造影剂后0~3rain的动态图像,并对获得的时间-强度曲线(TIC)进行分析,参数包括上升支斜率(A)、下降支斜率(a)、曲线下面积(AUC)、达峰时间(TTP)、达峰强度绝对值(DPI)。结果与正常组比较,第2组和第4组的A、a明显减小(P〈0.05),AUC显著增大(P〈0.05),TTP明显延长(P〈0.05),第3组和第5组仅表现为A减小(P〈0.05),各组的DPI差异均无统计学意义;与第3组和第4组比较,第2组AUC增大(P〈0.05),TTP延长(P〈0.05)。结论超声造影能较早反映低温条件下兔肾皮质血流动力学变化,对低温所致肾功能损害的早期诊断有-定的应用价值。Objective To explore the value of contrast-enhanced ultrasound(CEUS) in the evaluation of the renal cortical perfusion in rabbits in low temperature conditions. Methods 20 New Zealand rabbits were randomly divided into five groups,4 rabbits in each group.. 1) with the first group as the control group (under the conditions of room temperature) :2)The second group was treated under - 15℃ - -23℃ for 8 hours 3)the third group was treated under - 15℃ - - 23℃ for 4 hoursi 4)the fourth group was treated under - 6℃ - - 14℃ for 8 hours: 5) the fifth group was treated under - 6℃ - - 14℃ for 4 hours. SonoVue was injected via the ear vein at a dose of 0. 1 ml/kg, the renal perfusion imaging was made, observing the dynamic image of 0- 3 min after the injection and analyzing the time-intensity curves(TIC) obtained from the following parameters .- the slope rate of ascending curve(A), the slope rate of descending curve(a) ,area under the curve(AUC), time to peak(TTP), and derived peak intensity(DPI). Results Compared with the control group,the A, a of the second and the fourth group were significantly decreased (P〈 0.05), while the AUC was significantly increased (P〈 0.05) and the TTP was significantly prolonged(P〈0.05),for the third and the fifth group,only the A was decreased (P〈0.05),and the DPI in each group showed no significant difference. Compared with the third and the fourth group, the AUC of the second group was increased and the TTP was prolonged (P〈0.05). Conclusions Contrast-enhanced ultrasound can reflect the earlier renal cortical hemodynamic changes in rabbits in the low-temperature conclitions,and is of great value for the early diagnosis of renal damage caused by low temperature.
