摘 要:目的探讨双源CT常见泌尿系结石成分的定性诊断价值以及与饮食、生活习惯、代谢异常的关系。方法收集临床怀疑泌尿结石的病例1256例,分别行双源CT单能量和双能量扫描,扫描后数据传至工作站,统计CT值差值、比值,对体外结石采用标准结石定性分析化学试剂进行分析;不同成分的CT值差值、比值比较行非参数Kruskal-Wallis检验分析,比较两种能量扫描的辐射剂量。结果单能和双能两种模式的辐射剂量比较不具有统计学意义。磷酸钙、草酸钙、胱氨酸、磷酸铵镁、尿酸在两种能量状态下差值均具有统计学意义(P<0.05),而CT值比值草酸钙与胱氨酸比较不具有统计学意义(P>0.05)。人群中患结石的概率与尿和血液生化等指标建立logistic回归方程,P=e3.409x1+1.27x0-0.068+0.014x3-0.182/1+e3.409x1+1.27x0-0.068+0.014x3-0.182。结论双源CT可用于常见五种结石成分的定性诊断。Objective To explore the qualitative diagnosis of dual-source CT on urinary stones and analyze the relation between stones and diet,lifestyle,metabolic disorders and diseases of urinary system.Methods Collection of 995 cases of urinary stones were performed dual-source CT scanning,the data transmitted to the work station,the stone were analyzed by qualitative analysis of chemical reagents.The CT value of the difference,ratio compared by nonparametric test,Kruskal-Wallis to compare the two energy scan radiation dose.Results Single energy and dualenergy comparison of dose was not statistically difference(P 〈0.05),the same as the CT value of the ratio of calcium.The difference of Calcium phosphate,calcium xalate,cystine,struvte,uric acid are statistically difference(P 〈0.05).The probability of suffering from stones P =e^3.409x1+1.27x0-0.068+0.014x3-0.182/1+e^3.409x1+1.27x0-0.068+0.014x3-0.182.Conclusion Dual source CT determine the stone size,location,urinary obstruction and qualitatively analyze of stone composition.
