高选迷切加保留胃窦浆肌层半胃切除术的实验研究

摘　　要：目的 探讨高选迷切加保留胃窦浆肌层半胃切除术抗胃肠返流的作用。方法 将21只杂种狗随机分为三组,各组分别行高选迷切加保留胃窦浆肌层半胃切除术、胃大部切除BillrothⅠ式吻合及胃大部切除加保留胃窦浆肌层吻合术,且在术中、术前和术后分别行肌电图、胆酸测定和病理组织学检查,并加以比较。结果 高选迷切加保留胃窦浆肌层半胃切除组肌电图和胆酸测定均无明显改变,吻合口炎症轻;胃大部切除加保留胃窦浆肌层吻合术组和胃大部切除BillrothⅠ式吻合组肌电图明显变直,胆酸测定较术前明显升高,吻合口炎症重。结论 高选迷切加保留胃窦浆肌层半胃切除术具有明显的抗胃肠返流作用。Objective To study on the anti-gastrointenstinal tegurgitation faction of highly selective vagotomy add pylorus and antroseromus-cular layer preserving hemigastrectomy. Methods 21 dogs random divided into three group were operated by highly selective vagotomy add pylorus and antroseromuscular layer preserving hemigastrectomy, Billroth I subtotal gastrectomy and add pylorus and antroseromuscular layer preserving hemigastrectomy respectively. We detected the anti-gastrointenstinal regurgitation faction in preoperation intraoperation and postoperation with elec-tromyogram choh'c acid and pathological examination respectively.Results No change of electromyogram, cholic acid and pathological examination was detected in highly selective vagotomy add pylorus and antroseromuscular layer preserving hemigastrectomy group. However, the choh'c acid of postoperative Billroth I subtotal gastrectomy add pylorus and antroseromuscular groups were higher than preoperation and the electromyogram became smooth. Conclusion The anti-gastrointenstinal regurgitation faction of highly selective vagotomy add pylorus and antroseromuscular layer preserving hemigastrectomy has function of anti-gastrointenstinal regurgitation distinctly.
