超声对胆囊息肉样病变的诊断价值

摘　　要：目的探讨不同病理类型的胆囊息肉样病变的超声声像图特点,通过对胆囊息肉样病变的超声诊断与病理结果分析,评价超声对该病的诊断价值。方法对80例胆囊息肉样病变患者的超声图像与术后病理诊断进行回顾性对照分析。结果本组80例胆囊息肉样病变患者中,胆固醇性息肉58例,炎性息肉4例,腺瘤样增生2例,单纯性腺瘤3例,乳头状腺瘤4例,胆囊腺肌性增生2例,粘膜息肉样变3例,腺瘤并局部不典型增生及癌变4例。在病理诊断为胆囊息肉样病变中合并慢性胆囊炎64例,合并胆囊结石29例。术后胆囊病理检查诊断符合率为97.5%。结论应用超声图像分析息肉大小、数目、蒂及基底宽窄、增长速度、局部胆囊壁改变及血流等情况,诊断及鉴别诊断灵敏而准确,并且方便对病人的动态观察及随访,对胆囊癌前病变及早期胆囊癌的预防和早期诊断有重要价值。Objective To investigate sonographic characteristics of polypoid lesions of the gallbladder of different pathology and evaluate the value of ultrasonagraphy in polypoid lesions of the gallbladder by analyzing ultrasonic diagnosis and pathologic results. Metbods To retrospectively analyze ultra- sound images in 80 patients with gallbladder polypoid lesions. Results Of all the 80 gallbladder polypoid lesions, 58 cases in cholesterol polyp,4 cases in inflammatory polyp, 2 cases in adenomatoid hyperplasia, 3 cases in simplex adenoma,4 cases in smei-papillary adenoma,2 cases in adenomyosis hyper- plasia of gallbladder,3 cases in mucosal polyps degeneration and 4 cases in adenoma complicated by local dysplasia and cancer. There were 64 patients in polypoid lesions of gallbladder combined by chronic cholecystitis by pathology, and 29 cases complicated by cholecystolithiasis. The coincidence of post- operative pathologic diagnosis in gallbladder lesions is 97.5 %. Conclusions The ultrasound images analysis of polyps sizes and number, the pedicle and the basal width, growth rate, local changes in the gallbladder wall and blood flow in the diagnosis and differential diagnosis, were sensitive and accu- rate. And through facilitating the dynamic observation and follow-up of patients, uhrasonagraphy was of significant value in the preventing and early di- agonsing of precancerous lesions of gallbladder and early gallbladder.
