集束化护理策略在预防心脏外科术后气管插管患者非计划拔管中的应用

摘　　要：目的探讨集束化护理在预防气管插管非计划拔管中应用的效果。方法采用的集束化护理措施包括加强风险评估,完善基础护理,有效固定呼吸机管路,有效约束与镇静,加强术前访视及健康教育。比较集束化护理实施前后非计划拔管的发生率。结果 172例患者中有5例发生非计划拔管,占2.91%,发生率比实施集束护理前下降(P<0.05)。结论采用气管插管集束化护理策略可减少气管插管非计划拔管的发生,减轻患者痛苦,提高护理质量。Objective To explore the effects of bundle care applied in patients with tracheal intubation after cardiac surgery to prevent unplanned extubation （ UEX）. Methods The care bundles included strengthening risk assessment, perfecting basic nursing, effectively fixing the ventilator tube, effective restraint and sedative treatment, strengthening preoperative visit and health education. The rates of UEX before and after implementing bundle care were compared. Results Among 172 cases receiving bundle care, there were 5 patients occurring unplanned extubation, and the rate was 2.9l % , which was statistically lower than before （ P 〈 0.05 ）. Conclusion Bundle care can reduce the rate of UEX for tracheal intubation patients, so as to alleviate their suffering and improve the quality of nursing.
