腹腔镜下小腿交通支静脉离断术治疗下肢静脉曲张疗效分析

摘　　要：目的探讨腹腔镜下小腿交通支静脉离断术(subfascial endoscopic perforator surgery,SEPS)治疗下肢静脉曲张合并小腿交通支静脉瓣膜功能不全的疗效。方法对17例原发性深静脉瓣膜功能不全并小腿交通支静脉瓣膜功能不全合并内踝区湿疹、溃疡的患者,行腹腔镜深筋膜下交通支静脉结扎术和大隐静脉高位结扎并抽剥术,观察其疗效。结果手术时间50～90min,平均70min。术后随访6～38个月,平均22个月,术后所有病人下肢色素沉着明显减轻,10例足靴区湿疹患者7～15d痊愈,6例足靴区溃疡的患者8～29d愈合。1例足靴区湿疹患者术后出现溃疡,经积极换药治疗,9d后愈合。结论腹腔镜下小腿交通支静脉离断术治疗下肢静脉曲张合并小腿交通支静脉瓣功能不全的疗效肯定,创伤小,恢复快,切口并发症少,美观,具有良好的临床应用价值。Objective To study the laparoscopic crus traffic from a vein broken the varix surgical treatment with a calf traffic venous valve the function is not complete curative effect. Methods Seventeen patients with primary deep venous valve insufficiency and the calf communicating branch vein valve insufficiency merge malleolus district eczema and ulcer were undertaken laparoscopic deep fascia under the traffic branch vein ligation and saphenous vein high ligation and stripping 90 min, with an average of 70 min. Cases were followed surgery. Results The operation time was 504 up for 6-38 months, with an average of 22 months, after operation, all the patients lower limbs significantly reduce pigmentation, 10 cases foot boots area eczema patients 7 to 15 days heal, 6 cases of foot ulcer area with boots 8-29 days healing. In 1 foot boots area the postoperative eczema appear ulcer, the positive change medicine treatment, nine days healing. Conclusion Laparoscopic traffic from the broken leg vein surgical treatment with calf traffic tion is not complete the curative effect of the affirmation, small trauma, faster recovery, incision less plications, beautiful, which has good value of clinical application.
