掌背动脉逆行岛状皮瓣的临床应用

摘　　要：目的探讨掌背动脉逆行岛状皮瓣修复手指皮肤缺损的方法。方法基于掌背动脉与指掌侧动脉存在交通支这一解剖基础,本组设计以此交通支为蒂,带第1或第2或第3或第4掌背动脉的手背逆行岛状皮瓣,修复食、中、环、小指掌、背侧皮肤缺损28例。结果 28例术后皮瓣全部成活,皮瓣色泽、厚度及质地与受区皮肤接近,手部外形及功能恢复满意,皮瓣两点辨别觉6 mm～9 mm。结论掌背动脉逆行岛状皮瓣修复手指皮肤缺损操作简单,有广泛的临床应用价值,是修复手指掌背侧近节或近2节指皮肤缺损较理想的方法。Objective To explore the metacarpal artery retrograde island flap to repair finger skin defect of method. Methods Based on metacarpal artery and refers to the palmax artery exist the anatomical basis with traffic, the design to a traffic for di, the first or the second or third with fourth metacarpal artery or the back of retrograde island flap, repair, food, and little finger ring, palm, back and sides of the 28 patients with skin defect. Results After 28 cases all flaps survived, color, and texture thickness flap with the affected area close to skin, hand appearance and function recovered satisfactorily, two distinguish distance of flap was 6 mm- 9 mm. Conclusion metacarpal artery retrograde island flap to repair finger skin defect simple operation have extensive clinical application value, is back CeJin fingers palm repair section or nearly 2 section refers to the method of ideal skin defect.
