摘　　要：目的:探讨分娩过程中引起新生儿锁骨骨折的有关产科因素。方法:对2001年1月1日～2010年12月31日该中心儿童医院住院确诊的409例新生儿锁骨骨折病例进行回顾性分析。同时研究近5年该院妇幼保健院住院分娩的15 913例活产婴儿中发现的11例新生儿锁骨骨折病例,研究其分娩过程以及引起骨折的相关因素。结果:产伤性新生儿锁骨骨折多在因其他疾病行X线检查时发现,男婴锁骨骨折发生率大于女婴,右侧锁骨发生率大于左侧;阴道难产新生儿锁骨骨折的发生率远高于阴道顺产和剖宫产,剖宫产新生儿锁骨骨折的发生率最低;新生儿锁骨骨折预后良好。结论:严格掌握剖宫产指征,提高剖宫产以及阴道助产技术水平,正确掌握分娩时机,可降低新生儿锁骨骨折的发生率。Objective： To explore the related obstetrical factors of birth clavicular fracture of neonates during delivery. Methods： A total of 409 neonates with clavicular fracture who were diagnosed definitely and treated in the hospital from January 1 st, 2001 to December 31 st, 2010 were analyzed retrospectively. At the same time, 11 neonates with clavicular fracture among 15 913 live babies who were born in the hospital in recent five years were researched, the course of delivery and the related factors of clavicular fracture were analyzed. Results： The most of the neonates with birth clavicular fracture were found during X - ray examination because of other diseases, the incidence of male infants was higher than that of female infants, the incidence of right clavicular fracture was higher than that of left clavicular fracture ; the in- cidence of clavicular fracture in neonates born by transvaginal difficult labor was significantly higher than those in neonates born by transvagi- nal natural labor and cesarean section; the incidence of clavicular fracture in neonates born by cesarean section was the lowest; the prognosis of neonates with clavicular fracture was good. Conclusion： Controlling the indications of cesarean section strictly, improving the technical levels of cesarean section and vaginal assistant labor, and mastering the delivery opportunity correctly can reduce the incidence of clavicular fracture in neonates.
