摘　　要：实施家庭医生服务制度的长远目标是使家庭医生成为社区居民的健康"守护人"和医疗费用的"守门人"。近期目标是围绕"保基本、强基层、建机制",根据居民健康需求和家庭医生的服务能力,将服务对象由贫困人群逐步向慢病管理对象、65岁以上老人和一般人群扩大;逐步实施社区首诊、预约门诊、双向转诊,实现费用控制、健康管理。长宁区在探索家庭医生制度方面,通过"四医联动"到社区家庭医生制度的重要转变,积累了许多有特色的经验。The long-term objective of family doctor Institutions implementation is to make the Family doctor become the door-keeper of community residents health and medical insurance control. However, its near objective is to expand the service population from the poor to those chronic people inspected and the old above 65 years old, even the ordinal population. Changning district has already developed a very unique experience model in exploring the family doctor Institutions.
