胃内适量注气法在腹腔镜胃溃疡穿孔缝合修补术中的应用效果

摘　　要：目的探讨胃内适量注气法在腹腔镜胃溃疡穿孔诊断和缝合修补术中的临床应用效果。方法将2005年5月～2011年12月我院105例腹腔镜下胃溃疡穿孔缝合修补术中50例采用胃内适量注气法设为试验组、55例未采用胃内适量注气法进行诊断和治疗设为对照组,但其中有6例胃后壁穿孔转为开腹手术,故对照组以49例计算,比较两组的手术时间、住院时间及不同穿孔病例的诊断时间。结果试验组胃窦前壁病例穿孔的诊断时间显著短于对照组,试验组胃小弯侧穿孔病例诊断时间与对照组比较差异无统计学意义,且试验组胃内适量注气后与对照组比较,显著缩短手术时间及住院时间,差异有统计学意义(P<0.05)。结论在腹腔镜下胃溃疡穿孔诊断和缝合修补术中胃内适量注气充盈后,胃向前方和下方膨胀,穿孔部位显露更清楚,便于寻找,手术时间相对缩短,达到微创效果。Objective To explore the effect of using suturing repair method of suitably injecting gas into the stomach in laparoscopic gastric ulcer perforation. Methods From May 2005 to December 2011, 105 cases of laparoscopic gastric ulcer perforation suture repair surgery in 50 patients with stomach the amount of gas injection method, set up for the test group, 55 cases of stomach right amount of air spargingdiagnosis as the control group, but 6 cases of gastric wall perforation converted to open surgery, so the control group to calculate the 49 cases, operative time, hospitalization time and different perforation cases the diagnosis time between two groups were compared. Results The diagnosis time of perforation of the gastric antrum anterior wall of the test group was significantly shorter than the control group, the lesser curvature side of the perforation cases diagnosis time of the experimental group had no significant difference with the control group, and the stomach of the experimental group, the amount of gas injection compared with the control group had significantly shorter operative time and hospital stay, the difference was statistically significant （P 〈 0.05）. Conclusion Because the stomach forward- downward expansion after suitably injecting gas through the stomach tube in laparoscopic gastric ulcer perforation, perforation site is easy to find, reveal more clearly, the operation time is shorter than previous method and further to achieve minimally invasive effect.
