摘 要:目的调查肺炎克雷伯菌(KPN)临床株产不同β-内酰胺酶株分布情况、表型特征及耐药现状,指导临床合理使用抗菌药物。方法采用VITEK-2Compact对肺炎克雷伯菌进行鉴定和药敏试验。结果 291株KPN表型主要分为3类:产ESBLs类、产获得性青霉素酶类、野生型;其中产ESBLs株占48.1%;产获得性青霉素酶类占35.1%;野生株占14.4%;此外产碳青霉烯酶菌株7株,占2.4%,产酶总阳性率为85.6%;产ESBLs菌株对多数抗菌药物的耐药性明显高于非产ESBLs菌株。结论应高度重视对KPN产酶株的监控,合理使用抗菌药物,控制耐药株的产生与扩散。OBJECTIVE To evaluate the distribution,phenotypes and the drug resistance profiles of different kinds of β-lactamase-producing Klebsiella pneumoniae isolated from clinic.METHODS VITEK 2 compact was used to identify K.pneumoniae isolates,antimicrobial susceptibility testing was performed.RESULTS The phenotypes of 291 isolates of K.pneumoniae mainly presented as the following three categories: ESBLs-producing strains(48.1%),acquired-penicillinase-producing isolates(35.1%),and wild type isolates(14.4%);there were 7 strains of carbapenemase-producing isolates,accounting for 2.4%;the total β-lactamases-producing rate was 85.6%;the drug resistant rates of ESBLs-producing isolates were significantly higher than those of non-ESBLs-producing isolates.CONCLUSION We should attach great importance to the monitoring of the enzyme-producing K.pneumoniae isolates.It is necessary to reasonably use antibiotics so as to control the emergence and the spread of drug resistant strains.
