艾滋病患者合并苍白密螺旋体感染的临床研究

摘　　要：目的探讨人类免疫缺陷病毒(HIV)和苍白密螺旋体(TP)混合感染的临床特点。方法收集45份HIV/TP合并感染血清和57份单纯HIV阳性血清和全血,进行淋巴细胞、血常规、肝功能等检测,分析两组实验室检测指标的差异。结果 HIV/TP合并感染患者外周血单核细胞数为(431.6±127.8)109/L,HIV感染者为(527.0±156.2)109/L,两组差异有统计学意义(t=2.116,P<0.05),两组间其他检测指标T(CD3+)细胞、CD4+、CD8+T淋巴细胞数、CD4+/CD8+比值、B淋巴细胞、NK细胞、白细胞计数、淋巴细胞、红细胞、血小板、血红蛋白、丙氨酸转氨酶、白蛋白、γ-谷氨酰转肽酶、总胆汁酸及总胆红素差异均无统计学意义。结论 HIV/TP合并感染患者外周血单核细胞显著低于单纯HIV感染者,测定HIV/TP患者外周血单核细胞变化,有助于评价合并感染患者感染的免疫状况。OBJECTIVE To study the clinical laboratory characters of coinfection of Treponema pallidum and HIV.METHODS A total of 45 serum coinfected with HIV/TP and 57 pure HIV positive serum and blood were collected,T-lymphocytes,liver function and blood routine test were determined by the regular immunological,biochemical and molecular biological methods,the differences in the laboratory test indicators were analyzed.RESULTS The peripheral blood mononuclear cell counts of the patients with HIV/TP coinfection was（431.6±127.8） 109/L,the patients with HIV infection（527.0±156.2）109/L,the difference was significent（t=2.116,P〈0.05）;there were no statistical differences in other indexes including T（CD3＋） cells,CD4＋ and CD8＋ T-lymphocyte counts,the ratio of CD4＋/CD8＋,the B lymphocytes counts,NK cells counts,leukocyte counts,lymphocytes,red blood cells,platelets,hemoglobin,alanine aminotransferase,albumin,γ-glutamyltranspeptidase,total bile acid and total bilirubin.CONCLUSION The peripheral blood mononuclear cell count is significantly lower in the patients with HIV/TP coinfection than in the patients with simple HIV infection,the determination of peripheral blood mononuclear cell count may contribute to the understanding of the immune status of the patients with coinfections.
