摘　　要：目的有效控制骨科外来手术器械的清洗消毒质量,保证手术患者的医疗安全。方法将外院送入医院的已清洗灭菌但未使用的器械64件设为对照组,随机抽取已使用的外来器械186件,分成手工清洗组(观察1组)、机械清洗组(观察2组)和预处理+机械清洗+人工检查干预组(观察3组),清洗后用10倍放大镜目测加隐血试验检查清洗质量。结果 4组器械的放大镜目测不合格率分别为18.46%、25.81%、20.97%、1.61%,差异有统计学意义(P<0.01);隐血试验阳性率分别为13.85%、16.13%、11.29%、0,差异有统计学意义(P<0.05)。结论骨科外来手术器械的清洗质量不容乐观,必须加强外来器械的管理,通过规范清洗消毒流程及加强质量控制以保证外来器械的清洗质量,从而保障手术患者的医疗安全。OBJECTIVE To effectively control cleaning and disinfection quality of loaner surgical instruments in orthopaedic department so as to ensure the medical safety of the patients.METHODS Totally 64 pieces of submitted instruments which have been cleaned and sterilized were selected as the control group,and 186 pieces of loaner instruments were randomly selected and divided into the manual cleaning group（observation group 1）,mechanical cleaning group（observation group 2）,and pre-disposal plus mechanical cleaning plus manual intervention（observation group 3）,the cleaning quality of the instruments was examined by ten times magnifier plus occult blood test.RESULTS The unqualified rates of the cleaning quality of the four groups of instruments measured by the magnifier were 18.46%,25.81%,20.97% and 1.61%,respectively,the difference was statically significant（P〈0.01）,while the positive rates of the cleaning quality measured by occult blood test were 13.85%,16.13%,11.29% and 0,the difference was statistically significant（P〈0.05）.CONCLUSION The current status of the cleaning quality of loaner surgical instruments in orthopaedics department is not optimistic.It is necessary to strengthen the management of the loaner instruments to ensure the cleaning quality as well as the medical safety through the standardized process of cleaning and disinfection and the strict quality control.
