摘　　要：目的探讨南昌市糖尿病患者医院-社区一体化规范化管理的效果。方法在南昌市系马桩卫生服务中心接受规范化管理的50例糖尿病患者入选管理组,广润门社区卫生服务中心未接受规范化管理的46例糖尿病患者入选对照组,采用问卷调查的方法调查糖尿病患者的糖尿病健康知识知晓率、血糖达标率、慢性并发症筛查率,并通过定期检测血糖、糖化血红蛋白等指标,评价南昌地区糖尿病患者规范化管理的效果。结果干预1年后,管理组的糖尿病健康知识知晓率、血糖达标率、生活质量评分均较对照组显著改善(P值均<0.05)。结论通过医院-社区一体化无缝式糖尿病管理方式,可以有效地帮助患者血糖达标,提高患者的糖尿病健康知识知晓率、血糖达标率、慢性并发症筛查率及生活质量,是切实有效的管理模式。Objective To study the effect of hospital-community integrated management on the patients with diabetes mellitus in Nanchang area. Methods Fifty patients with diabetes in Ximazhuang Community Health Center were selected and managed with the hospital-community integrated model. Forty-six patients in Guangrunmen Community Health Center without standardized management were enrolled as cotrol. The questionnaires were used to investigate the diabetes health knowledge, blood glucose control and chronic complications. Some serum indices, such as blood glucose and glycosylated hemoglobin were periodically detected to evaluate the effect of standardized management for the patients with diabetes in Nanchang area. Results The health knowledge on the diabetes, blood glucose control and quality of life were significantly improved one year after the hospital-community integrated management （all P 〈0. 05）. Conclusion The hospital-community integrated management is more effective in controling blood glucose. It helps patients with diabetes to know more health knowledge, increases screening rate of diabetets related chronic complications, and improves quality of life.
