摘  要:目的探讨T2DM患者胰岛分泌功能与能量代谢相关参数的关系。方法将200例新诊断的T2DM患者分为胰岛β细胞功能指数(HOMA-β)低组和HOMA-β高组,比较两组间呼吸商(RQ)、非蛋白呼吸商(NPRQ)、静息能量消耗(REE)及胰岛素抵抗指数(HOMA-IR)、早相胰岛素分泌指数(△I30/△G30)、胰岛素分泌曲线下面积(AUCIns)、BMI。结果低组的RQ、NPRQ、HOMA-IR、△I30/△G30低于高组,NPRQ与BMI、HOMA-β呈正相关,REE与BMI呈正相关,与年龄呈负相关。结论胰岛β细胞功能较低的T2DM患者NPRQ较低,机体能量可能主要由脂肪代谢提供。Objective To study the relationship between the β-cell function and energy metabolism indexes of type 2 diabetic patients. Methods 200 newly diagnosed type 2 diabetes mellitus (T2DM) patients were divided into 2 groups according to their HOMA-β levels. Indirect calorimetry was used to evaluate the rest energy expenditure (REE), respiratory quotient (RQ) and nonprotein respiratory quotient (NPRQ) of the two groups. Homeostasis model assessment was used to evaluate basal insulin secretion (HOMA-β) and insulin resistance (HOMA-IR). △I30/△G30 and AUCIns (area under the curve of insulin) were used to evaluate early phase insulin secretion. Results RQ, NPRQ, HOMA-IR and △I30/△G30 were lower in the group with lower HOMA-~ NPRQ showed a positive correlation with BMI and HOMA-β, and REE showed a positive correlation with BMI and a negative correlation with age. Conclusion Type 2 diabetic patients with poor β-cell function have a lower NPRQ, and their energy expenditure may depend on the lipometabolism predominantly.
