摘 要:目的研究中枢神经系统(central nervous system,CNS)畸形合并羊水过多胎儿产尿率(urine production rate,UPR)的改变情况。方法选取58例正常单胎妊娠的胎儿作为正常对照组,选择同期连续性检测的20例CNS畸形合并羊水过多的胎儿作为病例组,并将其中的5例无脑畸形列为CNS畸形亚组。应用三维超声虚拟器官计算机辅助分析(virtual organ computer-aided analysis,VO-CAL)技术分别测量各组胎儿的膀胱体积并计算胎儿UPR,经过Z-score转换消除孕周影响,测得的病例组Z值分别与对照组进行比较。结果 CNS畸形组和无脑畸形组胎儿UPR与对照组均无明显差异(均P>0.05)。结论无论是无脑畸形还是其他类型,CNS畸形胎儿UPR正常,即CNS畸形合并羊水过多并非由于胎尿生成增多所致。Objective To evaluate the changes of fetal urine production rate (UPR) in fetuses with central nervous system(CNS) defects complicated with polyhydramnios. Methods 58 normal singleton fetuses was selected as the control group. 20 consecutive fetuses with CNS defects complicated with polyhydramnios was selected as the case group, among whom, 5 fetuses with anencepbalia were consid- ered as the subgroup of CNS defects. Fetal bladder volume was measured in each group using virtual or- gan computer-- aided analysis(VOCAL) technique, and then fetal UPR was calculated. To adjust for ges- tational age, UPR was transformed to Z--score. Compared the Z--score of the case group to the control group. Results Fetal UPR of CNS defects group and anencephalia group did not differ from that of the control group (P 〉 0.05). Conclusions No matter what it is, anencephalia or other types, fetal UPR in CNS defects is normal. It is not the increase of urine production that causes polyhydramnios in CNS de- fects.
