2型糖尿病伴高血压联合降压方案的临床研究

摘　　要：目的探讨联合降压治疗2型糖尿病伴高血压的临床疗效。方法研究对象为2007年9月～2010年12月某院收治的2型糖尿病伴高血压患者76例,其中硝苯地平治疗组设为对照组(27例),依那普利和络活喜联合治疗组设为观察组(49例),比较分析两组患者临床疗效及并发症情况。结果观察组总有效率为87.7%;对照组总有效率为51.8%,两组差异有统计学意义(P﹤0.05);治疗前观察组合并糖尿病肾病9例、冠心病11例;对照组合并糖尿病肾病5例、冠心病7例。治疗后观察组糖尿病肾病好转率66.7%,冠心病好转率27.7%;对照组糖尿病肾病好转率25.0%,冠心病好转率28.6%,两组差异有统计学意义(P﹤0.05)。结论在2型糖尿病伴高血压的治疗中联合降压治疗效果显著,安全可靠,值得临床上推广应用。OBJECTIVE To investigate the combined antihypertensive therapy in type 2 diabetes with hypertension patients. METHODS 76 subjects with type 2 diabetes and hypertension, were selected from September 2007 - December 2010 in our hospital. Control group received nifedipine （27 cases）, and treatment group received enalapril and amledipine （49 cases）. The clinical efficacy and complications were compared. In the observation group, the total effective rate was 87.7%, and was 51.8% in the control group. There was significant difference between the two groups （P〈 0.05） ; Before treatment, 9 eases had diabet- ic nephropathy, and 11 patients had coronary artery disease complicated with diabetic nephropathy in the treatment group; In the control group, 5 cases had diabetic nephropathy, and 7 eases had coronary heart disease. After the treatment, improved rates of diabetic nephropathy and coronary heart disease in observation group were 66.7% and 27.7%, respecitvely; In the con- trol group, improved rates were 25% and 28.6%, respectively. There was significant difference in the improve rate between the two groups （P 〈 0.05）. CONCLUSION Combined antihypertensive therapy effect has favorable effect in treatment of type 2 di- abetes with hypertension, safe and reliable, which is worthy of clinical application.
