168例彝族2型糖尿病患者自我管理现状调查

摘　　要：目的探讨168例凉山州彝族2糖尿病患者的自我管理水平及影响因素。方法采用焦虑自评量表(Self-Rat-ing Anxiety Scale,SAS)、抑郁自评量表(Self-Rating Depression Scale,SDS)、糖尿病自我管理水平问卷,以及自制的基本情况问卷,对2011年1～12月我科治疗的168例彝族2糖尿病患者进行问卷调查。结果自我管理各维度得分:饮食(3.58±1.80)分,运动(2.15±1.79)分,血糖监测(0.78±0.54)分,足部护理(0.38±0.16)分,遵医嘱用药(5.36±1.72)分。168例患者中,吸烟者65人,占38.69%,其中平均每天吸烟14.75支。职业因素影响饮食、运动及遵医嘱用药自我管理;文化程度为血糖监测、运动、遵医嘱用药的影响因素;抑郁及焦虑情绪与血糖监测自我管理有关;医疗方式主要影响彝族糖尿病患者的遵医嘱用药管理。结论凉山州彝族2糖尿病患者自我管理能力存在严重的不足,职业、文化程度、抑郁焦虑情绪及医疗方式等是其主要影响因素。Objective To investigate self-management levels of type 2 diabetic patients of Yi-people as well as it＇s influencing factors in Hangshan Yi Autonomous Prefecture. Methods One hundred sixty-eight patients with type 2 diabetes of Yi-people were investigated by using methods of consulting questionnaire, Diabetes Self-efficacy Questionnaire, Self-Rating Depression Scale （SDS） and Self-rating Anxiety Scale （SAS）. Results The scores of the self-management subseales were 3.58±1.80 for diet,2.15 ±1.79 for exercise,0.78 ±0.54 for glucose monitoring,0.38±0.16 for foot care,5.36±1.72 for medication （range=0-7）. In 168 patients 65 （38.69%） were smokers who smoked average 14.75 cigarettes per day. Patients＇ occupations influenced diet, exercise and medication. Education level were the influential factors of glucose monitoring, exercise and medication. Depression and anxiety influenced the glu- cose monitoring. Also, the medical treatment patterns were the major influential factor for medication. Conclusions The current condi- tion of self management in type 2 diabetic patients of Yi-people in Liangshan Yi Autonomous Prefecture was not ideal. The occupations, education level, depression and anxiety, and medical treatment pattenls were the main influential factors.
