常规治疗基础上加用ω-3鱼油脂肪乳治疗老年重症肺炎疗效观察

摘　　要：目的观察常规治疗基础上加用ω-3鱼油脂肪乳对老年人革兰阴性菌感染导致重症肺炎的临床疗效。方法31例病原学确诊为革兰阴性菌感染的老年重症肺炎患者分为试验组15例和对照组16例,均行气管切开插管,应用有创呼吸机辅助通气。对照组仅给予常规治疗,试验组在常规治疗的基础上加用ω-3鱼油脂肪乳静脉滴注。观察两组疗效。结果试验组有效率为80.0%,高于对照组的37.5%,试验组机械通气时间为(122.3±15.7)h,短于对照组的(183.4±12.8)h,差异均有统计学意义(P<0.05)。脱机成功率试验组为73.3%(11/15),对照组为56.3%(9/16),差异无统计学意义(P>0.05)。试验组治疗后血降钙素原水平低于对照组及治疗前(P<0.05),对照组治疗后与治疗前比较差异无统计学意义(P>0.05)。结论ω-3鱼油脂肪乳对老年人革兰阴性菌感染的重症肺炎有良好辅助治疗作用。Objective To observe the efficacy of conventional therapy plus co-3 fish oil fat emulsion for elderly severe pneumonia caused by gram-negative bacteria. Methods Thirty-one cases with severe pneumonia caused by gram-negative bacteria were ran-domly divided into observation group（ n = 15 ） and control group（ n = 16）. Both groups were treated with traeheotomy tube and invasive mechanical ventilation. The control group was treated with conventional therapy while the observation group was treated with conventional therapy plus intravenous injection of oJ-3 fish oil fat emulsion. The curative effects were compared between the two groups. Resuits The effective rate of the observation group was 80.0% that was higher than that of the control group （37.5%）. The duration of mechanical ventilation was（ 122.3±15.7 ） hours that were shorter than that of the control group（183.4±12.8） hours. The differences were statistically significant（ P 〈 0.05 ）. The successful weaning rate of the observation group was 73.3% that was higher than that of the control group （56.3%） , but the difference was not statistical significance. In the observation group, serum procalcitonin （PCT） level after treatment was significantly lower than that before treatment（ P 〈 0.05 ） while there were no significant differences before and after treatment in the control group. Moreover, serum PCT level of the observation group were significantly lower than those of the control group after treatment（ P 〈 0.05 ）. Conclusions w-3 fish oil fat emulsion is a good adjuvant therapy for elderly severe pneumonia caused by gram-negative bacteria.
