交锁髓内钉早期动力化治疗对骨折患者愈合情况及相关血清因子的影响

摘　　要：目的研究交锁髓内钉早期动力化治疗对骨折患者愈合情况及相关血清因子的影响。方法将2008年3月～2011年10月本院采用交锁髓内钉进行治疗的104例骨折患者随机分为对照组52例和观察组52例。对照组治疗后给予常规干预,而观察组则在对照组的基础上早期进行动力化干预治疗。将两组患者的骨折愈合情况、并发症发生率及治疗前后的骨折愈合相关血清因子进行检测及比较。结果观察组愈合时间短于对照组,骨不连发生率、延迟愈合发生率、并发症发生率均低于对照组,差异有统计学意义(P<0.05);观察组动力化治疗后不同时间段的血清OPG、BGP及ALP水平均高于对照组,sICAM-1及sVCAM-1水平均低于对照组,差异有统计学意义(P<0.05)。结论早期动力化治疗可促进骨折交锁髓内钉治疗患者的愈合,且其对骨折愈合的相关血清因子也有积极明显的影响作用。Objective To study the influence of early motorization treatment of interlocking intramedullary nail on healing situation and related serum factors of fracture patients. Methods 104 cases of fracture patients who were given interlocking intramedullary nail treatment in our hospital from March 2008 to October 2011 were randomly divided into the control group and the observation group, each group had 52 cases. The control group was given conventional intervention after treatment, the observation group was given early motorization intervention treatment on the basis of the control group. Then the fracture healing situation, incidence of complications and fracture healing related serum factors levels before and after treatment between the two groups were detected and compared. Results The healing time of the observation group was shorter than that of control group, incidence of nonunion, delayed union and complications of the observation group were all lower than those of the control group, the differences were statistically significant （P 〈 0.05）; serum OPG, BGP and ALP at different times after motorization treatment in the observation group were all higher than those of the control group, sICAM-1and sVCAM-1 levels were all lower than those of the control group, the differences were statistically significant （P 〈 0.05）. Conclusion Early motorization treatment can improve the healing of fracture patients treated with interlocking intramedullary nail, and it also has significant influence on fracture healing related serum factors.
