腰椎后路减压术后功能恢复及稳定性相关因素分析

摘　　要：目的分析影响腰椎后路减压术后功能恢复及脊柱稳定性的危险因素。方法回顾性分析112例腰椎后路减压术治疗的患者临床及随访资料,分析功能恢复和脊柱稳定性相关因素。结果功能改善优良率为86.61%。术后2周、3个月和6个月位移和成角差异有统计学意义(P<0.05)。不同病程长度、术前JOA评分、手术椎板数的功能恢复欠佳率差异有统计学意义(P<0.05),不同年龄段、病程长度、手术椎板数、手术小关节数△C差异有统计学意义(P<0.05),不同病程长度、手术椎板数、手术小关节数△E差异有统计学意义(P<0.05)。病程、术前JOA评分是功能恢复的危险因素(P<0.05),病程、手术椎板、手术小关节是脊柱稳定性的危险因素(P<0.05)。结论腰椎后路减压术前无失稳存在的患者术后随访时间内效果均较满意,病程、术前JOA评分、手术椎板数、手术小关节数与术后功能恢复和稳定性有关。Objective To analyze the risk factors of functional recovery and spine stability after tumbar posterior decom pression. Methods A retrospective analysis of 112 cases of lumbar posterior decompression for the treatment of patients with clinical and follow-up data and the analysis of functional recovery and spine stability were done. Results The excellent rate of improvement was 86.61%. Displacement and the angular difference after 2 weeks, 3 months and 6 months were statistically significant （P 〈 0.05）. Different course length, preoperative JOA score, and functional recovery of the surgery the laminectomy number of poor rate difference were statistically significant （P 〈 0.05）; different ages, length of course, the number of surgical laminectomy and the number of surgery small joints of A C difference were statistically significant （P 〈 0.05）; duration of different length, the number of surgical laminectomy surgery the number of small joints of AE difference were statistically significant （P 〈 0.05）. Duration of disease, preoperative JOA score were a risk factor for functional recovery （P 〈 0.05）; duration of disease, surgical laminectomy, small joints of the surgery were the risk factors of the spine stability （P 〈 0.05）. Conclusion The posterior lumbar decompression without instability satisfaction effect compared with the post- operative follow-up time, duration, and preoperative JOA score, the number of surgical laminectomy surgery the number of small joints and postoperative functional recovery and stability.
