改良脐静脉插管术在危重新生儿治疗中的应用

摘　　要：目的探讨改良脐静脉插管术在危重新生儿治疗中的应用。方法75例危重新生儿接受脐静脉置管术，术前估计置管长度，将导管缓慢插入脐静脉，当导管插入达到预计长度时，用预先充好生理盐水的注射器回抽有血，用外科0号非吸收丝线双重结扎固定导管，术后行床边胸片检查确定管端位于下腔静脉内。结果75例中2例置管失败，其余73例置管成功，留置时间为3-10d，73例均无出现发热等表现，4例拔管后管端送培养有细菌生长．根据药敏选用抗生素抗感染治疗有效，均无出现出血、血管栓塞、空气栓塞、坏死性小肠结肠炎等并发症。结论改良脐静脉置管术操作简单、直观、方便，有利危重新生儿病情的监测与治疗，具有重要的临床意义。Objective To investigate the application of umbilical vein catheterization in healing the severe newborn infant. Method there are seventy five severe newborn infant who accept umbilical vein catheterization. To estimate the length of Catheterization preoperative. Take the catheter slow- moving inset the umbilical vein, when catheterism to prediction length, use injector which be full of normal saline pumpbaek to have blood, fix the catheter with double zero non-absorb silk suture in surgery. Postop have a bebside sternum to sure the end of the tube located inferior vena cava. Result there ara two sample failure in umbilical vein catheterization, there are seventy three sample succeed in umbilical vein catheterization. The time to detain is three to ten days. The seventy three sample haven＇t a temperature and so on. There are two sample the end of tube to raise have bacterium groeth. Adoption antibiotics to anti-infection according to drug sensitivity in effect. All of them do not appear complication, for example, bleeding, blood vessel embolism, atmosphere embolism, necrotizing enterocolitis and so on. Conclusion Reforming umbilical vein catheterization operation is simple, direct and convenient. It is in favour of monitoring and treatment of the severe newborn infant. It is important in clinical.
