乙肝患者血清总胆汁酸、前白蛋白与相关酶学指标联合检测的临床价值分析

摘　　要：目的检测乙型肝炎(HB)患者血清总胆汁酸(totalbileacide,TBA)、前白蛋白(prealbumin,PA)及肝功能酶学指标[γ谷氨酰转肽酶(γ-glutamyltranspetidase,GGT)、丙氨酸氨基转移酶(glutamic-pyruvic transaminase,ALT)、天门冬氨酸氨基转移酶(Aminotransferase,AST)、碱性磷酸酶(Alkaline phosphatase,ALP)]水平变化规律,探索TBA、PA与肝功能酶学指标联检在HB实验诊断中的临床价值。方法选择门诊和住院乙型病毒性肝病患者124例,正常对照组50例,用全自动生化分析仪测定血清TBA、PA与肝功能各酶学指标含量。结果各组TBA及GGT、ALT、AST、ALP水平高于健康对照组,差异有统计学意义(P<0.05)。而PA、CHE均较正常对照组降低,差异有统计学意义(P<0.01)。相关性分析显示,TBA、PA分别与CHE、GGT、ALT、AST、ALP水平呈正相关,差异有统计学意义(P<0.05)。结论血清TBA、PA及肝功能酶学指标联检对肝病的临床诊断、鉴别诊断、病情判断有重要的临床参考价值。Objective To explore the clinical value of diagnosis of HBD by unite detecting the changes of total bile acid（TBA）,prealbumin（PA） in serum of Hepatitis B disease（HBD） and hepatic function enzymology such as γ-glutamyltranspetidase（GGT）,glutamic-pyruvic transaminase（ALT）,Aminotransferase（AST）,Alkaline phosphatase（ALP）.Methods The levels of TBA,PA and hepatic function enzymology were investigated in 124 HBD patients and 50 healthy control group by automatic biochemical analyzer.Results The levels of TBA and GGT,ALT,AST,ALP were significantly higher in HBD than those in the healthy control group（P〈0.05）.However,PA and CHE were lower than those in the healthy control group（P〈0.01）.At the same time,a significantly positive correlation was observed between the levels of TBA,PA and CHE,GGT,ALT,AST,ALP（P〈0.05）.Conclusion Unite detection of TBA,PA and hepatic function enzymology plays an important value in clinical diagnosis,analysis and prognosis of HBD.
