围手术期心理干预对肝癌TACE治疗患者的影响

摘　　要：目的探讨围手术期心理干预对肝癌TACE治疗患者的影响。方法回顾性分析2009年5月～2010年10月间接受TACE治疗的肝癌患者100例,随机分为对照组和观察组。对照组实施常规TACE手术护理,观察组在对照组护理基础上给予心理护理和健康教育干预,比较两种方法的临床应用效果。结果⑴观察组术中焦虑、恶心、呕吐发生率显著低于对照组(P<0.05、P<0.05、P<0.001)。⑵观察组术中收缩压、心率明显低于对照组,差异有统计学意义(P<0.01、P<0.01);然而两组术中舒张压比较无统计学意义(P>0.05)。结论围手术期心理护理和健康教育干预可以降低肝癌TACE治疗患者的应激反应,减少术中副反应的发生率,有利于手术的进行。Objective To assess the effect ofperi-operation period psychological intervention in patients with primary hepatocellular carcinoma receiving TACE treatment. Methods A retrospective analysis was carded out in 100 patients with primary hepatocellular carcinoma receiving the first time TACE treatment from May 2009 to October 2010 treated in our hospital. These patients were randomly divided into two groups： 50 cases given only basic nursing care were set as the control group, 50 cases of these patients received psychological care and health education besides basic nursing care were set as the observation group. The clinical efficacy was analyzed in the two groups. Results （1） The incidence rate of anxiety, nausea and vomiting in the observation group was lower than those in the control group, with significant difference （P〈0.05, P〈0.05, P〈0.001）. （2） The systolic blood pressure （SBP） and heart rate （HR） in the observation group were significantly lower than the control group （P〈0.01, P〈0.01）. However no significant change of the diastolic blood pressure （DBP） was detected between the two groups （P〉0.05）. Conclusion Peri-operation period psychological intervention could be effective in patients with primary liver cancer and dealing with intervention to reduce the stress response,the incidence of intraoperative side effects and promoting to postoperative rehabilitation.
