摘　　要：目的探讨TGF-β1局部注射联合经皮自体骨髓移植对骨折愈合的促进作用。方法预制新西兰大耳白兔骨延迟愈合模型,将40只新西兰白兔随机分为A、B、C和D组,每组10只。每组分别在骨缺损模型建立后5天和10天于骨延迟愈合区注入自体血液2ml加生理盐水1ml;自体红骨髓2ml加生理盐水1ml;自体血液2ml加TGF-β11ml(100μg);自体红骨髓2ml加TGF-β11ml治疗。术后4周行组织学观察和骨痂内骨系细胞立体定量分析;术后8周行骨几何参数和骨密度(骨痂处)测定,骨生物力学测试和单位质量骨痂中钙含量测定。结果 D组成骨细胞的平均体积密度、骨痂厚度改变、钙含量、骨折愈合处的最大抗弯强度、极限应力、极限刚度和极限负荷时的能量吸收,均非常显著性高于A组(P<0.01),显著性高于B组和C组(P<0.05)。结论 TGF-β1局部注射联合经皮自体骨髓移植,是治疗骨折延迟愈合的一种有效方法。Objective To explore the local injection of transforming growth factor β1（TGF-β1） combined with percutaneous bone marrow autograft on treatment of delayed union of fracture.Methods The model of delayed union in 40 New Zealand white rabbits’ radius fracture had been made and randomly divided into groups A,B,C,and D（n=10）.Group A were treated with autologous blood 2ml and normal saline 1ml;Group B were treated with autologous red marrow 2ml and normal saline 1ml;Group C were treated with autologous blood 2ml and TGF-β1 1ml（100ug）;Group D were treated with autologous red marrow 2ml and TGF-β1 1ml（100 μg）.All of them were done in the region of delayed union of fracture in the time of 5th and 10th days after the bone defects models were made.Histological observation and quantitative analysis of osteocytes in the callus were made in the 4th weeks after treating.Bone geometric parameters,bone mineral density,bone biodynamics test and unit mass of calcium content of callus were determined in the 8th weeks after treatment.Results The osteoblast average volume density,the changes of callus thickness,calcium content,the maximum bending strength,limit stress,ultimate stiffness and energy absorption as at ultimate loading on fracture healing were very significant higher in group D than group A（P〈0.01）,significant higher than group B and group C（P〈0.05）.Conclusion The treatment of delayed union of fracture by local injection of TGF-β1 combined with percutaneous autologous bone marrow transplantation is an effective method.
