摘 要:目的应用超声血流向量成像(VFM)技术评价急性冠脉综合征(ACS)患者收缩期左心室内血液流场的特点。方法选取ACS患者32例和健康志愿者40例,采用ALOKAα10超声诊断仪行常规超声心动图检查,采集标准心尖三腔心切面连续三个心动周期的彩色血流动态图像,脱机应用VFM软件处理并分析测量收缩期左心室涡流大小、位置、流量、强度等特征指标,并对比ACS患者与正常人左心室涡流变化的差异。结果ACS组收缩期涡流持续时间延长;等客收缩期至快速射血期涡流面积显著大于对照组,涡流中心位置靠近左室流入道中部,涡流强度下降;ACS组减慢射血期涡流呈现流量、强度均逐渐减小的规律,而面积无明显变化。结论ACS患者收缩期涡流特征与正常人存在特征性差异,VFM技术提供了评价ACS患者左室内血液流场变化及研究左室整体功能的一种新方法。AIM To investigate the characteristics of left ventricular (LV) systolic intracavitary flow field of patients with acute coronary syndrome (ACS) by vector flow mapping (VEM). METHODS Thirty-two ACS patients and 40 healthy volunteers were enrolled in this study as ACS group and control group respectively. ALOKA a10 ultrasonic diagnostic system was employed. All cases were examined by two-dimensional echocardiography first, and the color Doppler images of standard apical three-chamber in three sequent cardiac cycles were acquired. Recorded loops were analyzed off-line by VFM software in DAS-RS1 system. The left ventricular systolic vortex parameters including systolic vortex duration, vortex area, center position, maximum vector speed, vortex volume and intensity were measured and compared between the groups. RESULTS In ACS group, the systolic vortex duration time was longer than in control group. During isovolumetric contraction and rapid ejection, among ACS patients, the vortex area was larger; the vortex center position was nearer to middle part of LV inflow tract; the vortex intensity was significantly lower. In slow ejection, most normal cases showed no vortex in LV, while most ACS patients (23/32) showed long-lasting vortex with progressively decreasing area, flow volume and intensity. CONCLUSION There were significant differences in LV systolic vortex characteristics between ACS patients and healthy ones. VFM could be regarded as a new approach in evaluation of intracavitary hemodynamic changes in clinical application.
