2009～2011年湖州市中心医院感染横断面调查分析

摘　　要：目的了解医院感染基本情况及相关因素,以加强医院感染的预防和控制。方法采取横断面调查法,进行床边逐个调查。结果 3次总医院感染现患率为5.82%,总例次现患率为5.94%。各年感染现患率、例次现患率比较差异均有统计学意义(P均<0.05)。下呼吸道是医院感染最主要的部位。重症医学科医院感染现患率均为最高,10个科室3年调查现患率差异有统计学意义(P<0.05或P<0.01)。3年感染标本送检率及检出病原菌菌株阳性率均呈上升趋势(P<0.01)。年龄、侵入性操作、重症监护、抗生素联用及住院时间与医院感染发生关系密切(P<0.05)。结论本医院感染现患率符合国内对三级甲等医院的要求,近3年现患率呈下降趋势,下呼吸道仍是主要的感染部位,加强危险因素控制,以利于防止医院感染发生。Objective To investigate the basic situation of nosocomial infection and related factors, in order to strengthen the prevention and control of nosocomial infections. Methods A cross-sectional survey method, the bedside one by one investigation. Results The 3 times total nosocomial infection prevalence rate was 5.82%, total cases prevalence rate was 5.94%. Each year prevalence rate of cases times the prevalence rate differences were statistically significant （P 〈 0.05）. Lower respiratory tract were the major site of nosocomial infection. Nosocomial infection prevalence rates of ICU were the highest. The prevalence rates of 3-year of 10 departments difference were statistically significant （P 〈 0.05 or P 〈 0.01）. 3-year infection rate of specimens sent and the positive rate of detection of pathogenic strains showed an upward trend （P 〈 0.01）. Age, invasive procedures, intensive care, antibiotics, joint and length of stay and nosocomial infection were closely related （P 〈 0.05）. Conclusion The nosocomial infection prevalence rate in accordance with domestic requirements in the hospitals of the past three years, the prevalence rate declined, the lower respiratory tract is still the major site of infection, strengthening the control of risk factors, in order to facilitate to prevent nosocomial infection.
