先天性苗勒氏管畸形的MRI诊断

摘　　要：目的 探讨MRI对先天性苗勒氏管畸形的诊断价值.方法 69例临床怀疑先天性苗勒氏管畸形的患者行MRI检查,回顾性分析MRI表现及诊断结果,与手术结果或临床诊断进行对照.结果 69例中先天性苗勒氏管畸形57例,12例无苗勒氏管畸形,其中10例子宫阴道正常,另2例为子宫肌瘤合并出血.MRI判断有无先天性苗勒氏管畸形及各种亚型的准确率、阳性预测值、阴性预测值、特异度、灵敏度分别为85.5%（59/69）,82.5%（47/57）,100%（12/12）,54.5%（12/22）,100%（47/47）.结论 MRI能较准确的鉴别各种苗勒氏管畸形,是诊断先天性子宫阴道畸形较为可靠的方法.Objective To investigate the diagnostic value of magnetic resonance imaging for congenital Mullerian duct malforma tion. Methods 69 patients suspected with Mullerian duct malformation underwent MRI examinations. MR imaging features of Mullerian duct malformation were retrospectively reviewed. MR imaging findings were compared with surgical or clinical data. Results Among 69 cases,there were 57 cases of congenital Mullerian duct malformation, 12 without congenital Mullerian duct malforma- tion,of which,uterus and vagina were normal in 10 cases and uterine fibroids with bleeding in 2 cases. In the diagnosis of congenital Mullerian duct malformation and various subtypes, the accuracy rate, positive and negative predictive values , specificity and sensitivity with MRI were 85.5%（59/69） ,82.5%（47/57）and 100%（12/12） ,54.5%（ 12/22）and 100% （47/,17）, respectively. Conclusion MRI can accurately identify various Mullerian duct malformation, which is a relative reliable method in diagnosing uterine and vaginal anomalies.
