摘  要:目的 探讨宝石能谱CT在手足肌腱损伤中的初步应用.方法 对因怀疑肌腱病变来本院行手足部能谱CT扫描的61例患者,选取具有最佳对比噪声比(contrast to noise ratio,CNR)的系列图像,运用多种后处理技术如VR、MPR、CPR等对比观察并记录双侧肌腱的走行、止点、外形及周围组织情况.结果 肿瘤、肌腱缝合或骨折术后23例,慢性损伤19例,骨折或关节脱位13例,骨肿瘤、突起或周围软组织肿瘤10例,肌腱断裂2例,先天畸形1例.诊断准确率为89.71%(61/68),除肌腱慢性损伤诊断准确率较低外(12/19),其余病变诊断准确率达100%.结论 宝石能谱CT能清晰显示肌腱断裂、缝合术后、畸形等,但对肌腱慢性炎性改变显示欠佳.Objective To evaluate the feasibility of spectral CT imaging in diagnosing the injuries of hand and foot tendon. Methods 61 patients who suffered from tendon diseases of hands or feet were scanned by spectral CT imaging. The images with optimal contrast to noise ratio{CNR} were selected and the tendon shape, post-processing techniques, such as VR, MPR and CPR. Results outline, endings and surrounding tissues were observed with many In 61 cases,there were 23 cases with post-operations of tumors, tendon suture or fractures,chronic injures in 19 cases,fractures or dislocations of joint in 13 cases,tumors or eminence of bone and soft tissue tumors in 10 cases,rupture of tendon ealcaneus in 2 cases and congenital malformation in 1 case. The total diagnostic accuracy was 89.71%(61/68) with CT. Most of the tendon diseases could be diagnosed satisfactorily except the chronic injures. Conclusion Tendons rupture,post-operation and malformation of hand and foot can be clearly showed by spectral CT imaging,but the chronic inflammation of tendons cannot be detected so well.
