IVF受精低下患者再次助孕的治疗对策

摘　　要：目的探讨体外受精-胚胎移植（IVF-ET）中受精低下患者再次助孕治疗的授精方式及其可行性。方法回顾性分析2007年1月至2009年7月在我院进行IVF治疗过程中受精低下（受精率≤30％）的患者共60周期。结果受精低下的患者再助孕治疗采用IVF方式授精，其正常受精率与同时期连续IVF周期相比，明显降低，差异具统计学意义（P〈0．01）；但优质胚胎率及临床妊娠率差异无统计学意义；采用卵胞浆内单精子显微注射（ICSI）方式授精，其优质胚胎率与同时期常规ICSI比较明显升高，差异具统计学意义（P〈0．01）；但正常受精率及临床妊娠率差异无统计学意义；再助孕时两种授精方式的临床结局相比较差异无统计学意义（P〉0．05）。结论IvF受精低下患者再助孕治疗时需综合评估后再决定授精方式。Objective To investigate the fertilization method and the feasibility by using ART for conventional IVF patients with low fertilization. Methods The fertilization methods and treatment result of the conventional IVF patients with low fertilization （fertility rate ≤ 30%） were retrospectively analyzed in our research center from Jan. 2007 to Jul. 2009. Results For the patients with low fertilization treated with repeated IVF, the fertilization rates were obviously lower than that of the successive IVF cycles at the same period （P〈0.01）, but there were no differences in the high quality embryo rate and pregnancy rate. After the patients with low fertilization received the ICSI treatment with repeated ART again, the good quality embryo rates were obviously higher than that of the ICSI cycles at the same period （P〈0.01）, but there was no difference between the normal fertilization rate and pregnancy rate, and no difference in clinical outcomes between the two fertilization methods （P〉0.05）. Conclusion About what methods of insemination adopted for the IVF patients with low fertilization is decided after an overall evaluation
