血清细胞因子与老年冠心病关系的研究

摘　　要：目的 探讨老年冠心病（CHD）患者不同状态下超敏C-反应蛋白（hsCRP）、白介素-6（IL-6）的变化.方法 采用酶联免疫吸附法检测稳定性心绞痛（SA）患者32例,不稳定性心绞痛（UA）患者28例,急性心肌梗死患者（AMI）18例及健康对照（HC）50例的超敏C-反应蛋白、白介素-6的水平.结果 各组hs-CRP分别为6.29±1.79 mg/L（SA组）,8.66±2.03 mg/L（UA组）,8.49±2.49 mg/L（AMI组）,2.09±0.59 mg/L（HC组）;IL-6分别为0.907（0.811～1.214）ng/L（SA组）,1.043（0.933～2.642）ng/L（UA组）,2.062（0.963～2.462）ng/L（AMI组）,0.264（≤0.221～0.364）ng/L（HC组）.病例组CRP、IL-6均高于对照组（P＜0.05）;组间两两比较,CRP、IL-6：UA组＞SA组（P ＜0.05）,AMI组＞SA组（P＜0.05）,而UA组与AMI组差异无显著性;CRP 与IL-6呈正相关（r=0.817,P＜0.001）.结论 冠心病患者存在细胞因子的改变,且病情越重,细胞因子水平越高,可能与其血管内皮炎性反应有关.Objective To explore the transformation of high - sensitivity C - reactive protein （ hs - CRP） and in-terleukin - 6 （ IL - 6 ） in different conditions of elderly coronary heart disease （CHD）. Methods hs - CRP and IL - 6 were examined in 32 patients with stable angina（SA） ,28 patients with unstable angina（ UA）, 18 patients with acute myocardial infarction（AMI） and 50 healthy control（HC） by enzyme linked immunosorbent assay. Results hs-CRP was 6.29 ± 1.79 mg/L（SA group） ,8.66 ± 2.03 mg/L（ UA group） ,8.49 ± 2.49 mg/L（ AMI group） ,2.09 ± 0.59 mg/L（ HC group）, respectively. IL - 6 was 0. 907 （0.811 - 1. 214 ） ng/L （ SA group）, 1. 043 （ 0. 933 ± 2. 642 ） ng/L （ UA group） ,2. 062 （ 0. 963 - 2. 462 ） ng/L （ AMI group）, 0. 264 （≤0. 221 - 0. 364 ） ng/L （ HC group ）, respectively. CRP and IL-6 were both higher in the first three groups than in HC group（ P 〈 0.05 ） ;CRP and IL-6 were both higher in UA group than in SA group（ P 〈 0.05 ） , and they were higher in AMI group than in SA group（ P 〈 0.05 ）. However,there was no significant difference between UA group and AMI group. There was a positive correlation be-tween CRP and IL - 6 （ r = 0.817, P 〈 0. 001 ）. Conclusion Transformation of cytokines exist in patients with coro-nary heart disease. The level of cytokines increases with the severity of the disease, which may relate to vascular endo-thelial inflammatory reaction.
