脑泡型包虫病的CT和MRI表现与病理关系

摘　　要：目的分析脑泡型包虫病的CT和MRI的影像表现,结合病理标本探讨其影像分型。方法回顾性分析经临床病理证实的12例脑泡型包虫病的CT和MRI表现,观察病变部位、形态、大小、实性和囊泡构成以及囊泡位置、形态和大小。结果 12例病变共检出49个病灶,直径0.5～4.4 cm,平均1.7 cm。2例单发,10例多发。额叶12个、顶叶16个、颞叶9个、枕叶4个、基底节区5个、小脑半球2个、脑干1个。病变呈实性、囊实性和囊性改变,囊泡散在分布病灶内或周边分布为主,呈圆形或类圆形,直径≤20 mm。脑泡型包虫病影像表现分为6型:实性肿块型、小囊泡-实性肿块型、大囊泡-肿块型、多囊泡型、单囊泡型和结节型。结论脑泡型包虫病CT和MRI具有一定特征性,MRI能够更好地反映病理影像特征。Objective To analyze the CT and MRI features of cerebral alveolar echinococcosis, and to investigate imge classification with pathologic sample. Methods Combining the clinical manifestations and pathological characteristics, the CT and MRI features of twelve patients with cerebral alveolar echinococcosis were analyzed retrospectively with emphasis on the location, shape, size of vesica. Results There were 49 lesions in twelve patients. The size of lesions varied from 0.5 cm to 4.4 cm with average size of 1.7 cm. Two cases had single lession and 10 cases had multiple lesions. 12 lesions located in frontal lobes, 16 lesions located in parietal lobes, 9 lesions located in temporal lobes, 4 lesions loca- ted in occipital lobes, 5 lesions located in basal ganglion, 2 lesions located in cerebellar hemispheres, 1 lesion located in brain stem. The lesions were solid masses with mixed cystic area or cyst, vesicas were distributed in lesions or around lesions with round or similar round shape, diameter of vesicas were lesser than 20 mm o There were 6 types of cerebral alveolar echinococcosis： solid mass without cyst, small cyst with a solid mass, large cyst with solid mass, muliple cysts, single cyst and nodules. Alveolar echinococcosis was eventually diagnosed. CT and MRI examinations were performed on 12 patients who had cerebral alveolar echinococcosis. Conclusion There are certain CT and MRI features of cerebral alveolar echinococcosis.
