摘 要:目的了解北京市某医院门诊处方常见问题,提高处方质量,规范处方管理,促进临床合理用药。方法随机抽取该院2011年1—12月10950张门诊处方,根据《处方管理办法》、《医院处方点评管理规范(试行)》以及《抗菌药物临床应用指导原则》进行点评、统计和分析。结果该医院门诊处方平均每张用药品种数为2.00种,抗菌药使用率平均为25.25%,注射剂使用率平均为18.8%。在10950张门诊处方中,不合理处方445张,不合格率为4.06%。其中,不规范处方216张,占不合格处方的48.54%;用药不适宜212张,占不合格处方的47.64%;超常处方17张,占不合格处方3.82%。结论该医院门诊处方基本合理,但尚有问题存在,仍需加强对处方的管理、审核及干预以进一步提高处方质量。Objective Understanding common problems in outpatient prescriptions in the hospital, improving the quality of prescriptions and standardizing the prescription management by analyzing outpatient prescriptions, so as to promote the rational use of drug. Methods Judging and analyzing 10950 random samples of outpatient prescriptions from Jan to Dec in 2011. According to measures for the administration of prescriptions, measures for the administration of prescriptions in hospital, and guiding principles for clinical application of antibacterial etc. Results There were 445 unreasonable prescriptions (4. 06percent), 216 nonstandard prescriptions (48.54percent), 212 inexact prescriptions (47. 64pereent) and 17 excess prescriptions in the 10950 random samples. Conclusion The outpatient prescriptions in the hospital is basically rational, but there were still problems; improving the quality of prescriptions and strengthen prescription examination is still necessary.
