摘　　要：目的探讨胸腰椎结核手术治疗方法及疗效。方法对22例胸腰椎结核患者术前采用化疗抗结核,根据患者结核病灶位置,分别从胸椎、腰椎前路、后路彻底清除结核病灶,植骨融内固定。结果 22例均获得随访,时间18个月。胸腰椎结核无复发,结核中毒症状消失,疼痛缓解。神经功能Frankel分级:术前B级1例,C级11例,D级10例,术后恢复到E级21例,D级1例。结论在系统化抗结核治疗的基础上对有手术适应证的患者,采用手术清除结核病灶,可缓解神经症状,有效控制或治愈结核。Objective To study the methods and curative effect of surgical procedure for thoracolumbar spinal tuberculosis. Methods 22 cases of thoraeolumbar spinal tuberculosis patients were treated by chemotherapy for anti-TB treatment before the operation. According to the site of thoracolumbar spinal tuberculosis, the individualized surgical lreatment was used by anterior/posterior focus clearance and bone graft with internal fixation. Results All cases were followed up for 18 months postoperatively. No case had a relapse, all tuberculosis symptoms disappeared and the pain alleviated. The neural function from 1 case of Frankel B grade, l lcases of Frankel C grade, and 10 cases of Frankel D grade improved to 21 cases of Frankel E grade and 1 case of Frankel D grade after the operation. Conclusions Individual operation with focus clearance based on systematic anti-TB treatment can alleviated the neurological symptoms ,effectively control the development of tuberculosis.
