摘　　要：目的评价第三代加长型Gamma钉治疗老年骨质疏松性股骨转子间骨折的疗效。方法对247例老年骨质疏松性股骨转子间骨折患者行第三代加长型Gamma钉固定,分析手术时间、术中失血量、并发症发生率,进行髋关节Harris评分。结果手术时间0.8～2.3(1.1±0.4)h,术中失血量320～921(514±234)ml。患者均获随访,时间4～34(14±8)个月。骨折愈合时间为术后2.6～4.9(3.8±0.9)个月。术后12周时髋关节功能Harris评分:优146例,良77例,可24例,优良率为90.3%。术后6例发生脑梗死,5例出现肺部感染,7例切口浅表感染。结论第三代加长型Gamma钉治疗老龄股骨转子间骨折,可早期功能锻炼,并发症发生率低,疗效满意。Objective To investigate the clinical outcomes of Gamma 3 long nail in the treatment of femoral intertrochanteric fractures in elderly with osteoporosis.Methods 247 cases with femoral intertrochanteric fractures were treated by Gamma 3 long nail.The operative time,intraoperative blood loss,fracture healing time,hip Harris scores,and postoperative complication were recorded and analyzed.Results The operation time was 0.8-2.3（1.1±0.4） h.Intraoperative blood loss was 320-921（514±234） ml.247 cases obtained a mean of follow-up of 4-34（14±8） months.All fractures got union in 2.6-4.9（3.8±0.9） months.The results were excellent in 146 cases,good in 77,and fair in 24,and the excellent and good rate was 90.3% according to Harris score system in 12 weeks.There were 6 patients with cerebral infarction,5 pulmonary infection,and 7 wound infection.Conclusions The elderly of femoral intertrochanteric with osteoporosis can achieve satisfactory hip function,low complication rate,early postoperative .
