摘　　要：目的:探讨鼻咽癌中医分型与影像学特征及疗效的相关性。方法:收集140例鼻咽癌患者,在治疗前行中医分型及CT、MRI检查,在治疗中观察近期疗效并长期随访。结果:血瘀型和血瘀痰凝型更易出现颅底骨质破坏(P<0.001)及咽旁间隙侵犯(P<0.01),痰凝型与血瘀痰凝型更易出现颈部淋巴结肿大(P<0.001)。放疗后3个月,血瘀型和血瘀痰凝型的残余病灶较肺热型及痰凝型大(P<0.05)。血瘀痰凝型的4年无进展生存率较非血瘀痰凝型低(P=0.004)。结论:不同中医分型间存在不同的影像学特征。血瘀痰凝型及血瘀型近期疗效较肺热型与痰凝型差,血瘀痰凝型远期疗效较非血瘀痰凝型差。Objective To explore the correlation of traditional Chinese medicine （TCM） syndrome differentiation with imaging characteristic and treatment effect of nasopharyngeal carcinoma. Methods Collecting 140 cases of untreated nasopharyngeal carcinoma. Prior to treatment, TCM syndrome typing, magnetic resonance imaging （MRI） and computed tomography （CT） were performed on all patients. Tumor assessments were evaluated at accumulated doses of 20 Gy, 40 Gy and 60 Gy, and at 3 months after radiotherapy. All patients had been followed up until September 30,2011. The results were statistically analyzed by SPSS. Results Blood stasis-phlegm congealment and blood stasis types were more prone to skull base invasion and paranasopharyngeal space invasion. Blood stasis-phlegm congealment and phlegm congealment types were more likely suffered from cervical lymph node metastases. Residual tumor size was smaller in lung heat and phlegm congealment types than in blood stasis and blood stasis-phlegm congealment types after 3 months of treatment. The four-year overall survival rate of blood stasis- phlegm congealment type is lower than the other types. Conclusions Different TCM syndrome types have different imaging features. The short-term treatment effect of blood stasis and blood stasis-phlegm congealment types were worse than lung heat and phlegm congealment types, while long-term efficacy of blood stasis-phlegm congealment type was worse than the other types.
