摘 要:目的利用超声动脉健康评估和速度向量成像技术检测系统性红斑狼疮SLE患者颈动脉弹性变化。资料与方法随机选取31例SLE患者与20名年龄匹配的志愿者,利用超声动脉健康评估软件测量颈动脉收缩期、舒张期内中膜厚度,并计算颈动脉内中膜厚度变化值和内径变化值;速度向量成像技术测量颈动脉壁短轴方向6个节段的收缩期最大速度、最大切向应变、最大切向应变率及各参数达峰时间。结果 SLE患者颈动脉舒张期内中膜厚度及其变化值明显高于志愿者(P<0.05),而颈动脉收缩期内中膜厚度及内径变化值与志愿者无明显差异(P>0.05);所有受试者颈动脉前壁、前外侧壁和后外侧壁收缩期最大速度高于后壁、后内侧壁和前内侧壁;与志愿者比较,SLE患者各节段收缩期最大速度、最大切向应变、最大切向应变率及各参数达峰时间均明显降低(P<0.05)。结论超声动脉健康评估和速度向量成像技术能发现SLE患者颈动脉弹性减低的线索。Purpose To explore the arterial health package and velocity vector imaging (VVI) technique in detecting elasticity of carotid artery in patients with systemic lupus erythematosus (SLE). Materials and Methods Thirty-three patients with SLE and twenty control subjects underwent carotid ultrasonography. Systolic IMT (IMTs) and diastolic IMT (IMTd) were measured, and variation of IMT ( A IMT) and diameter ( A D) were calculated. VVI was used to measure maximum radial velocity (Vmax), maximum tangential strain (S), and maximum tangential strain rate (SR) in 6 segments of carotid arteries and time to peak of these parameters were also recorded. Results IMTd and A IMT of SLE group were higher than those of control group (P 〈 0.05), but IMTs and A D had no significant difference between two groups. Vmax of anterior wall, anterolateral wall and posterolateral wall were higher than those of posterior wall, posteromedial wall and anteromedial wall. Vmax, S and SR of SLE group were lower than those of control group. VP, SP and SRP had the same trend. Conclusion VVI and arterial health package can be used to detect early decrease of elasticity of carotid artery in patients with SLE.
