摘  要:[目的]探讨采用自主创新技术(凹侧双棒术、改良胸廓成型术和自体同种异体骨混合移植术)矫治重度僵硬性特发性脊柱侧弯的效果。[方法]回顾性分析2007年8月~2012年6月期间采用创新技术矫治的重度僵硬性特发性脊柱侧弯患者74例,其中男24例,女50例;年龄14~39岁,平均18岁。实验组36例采用创新技术一次性后路矫治,对照组38例采用常规后路钉棒矫治,比较两组手术前后的主侧凸冠状面Cobb角、主侧凸顶椎偏距、身高、手术时间、出血量等8项指标。[结果]平均随访47个月,实验组主弯Cobb角矫正率平均为62.50%,对照组为42.10%,主凸顶椎偏距矫正率平均为45.20%,对照组为36.20%,身高增长平均为7.0 cm,对照组为3.0 cm,以上指标两组间比较均有显著性差异(P<0.05)。两组手术时间、出血量比较无明显差异(P>0.05)。[结论]创新技术可提高重度僵硬性特发性脊柱侧凸的矫正率,安全可靠。[ Objective] To discuss the contribution in increase the results for correction of the severe and rigid idiopathic scoliosis with innovation technology. [ Methods ] The retrospective contrast method was used in the study. Thirty - six cases which received operation of double clubs for correction in concave side were investigated in study group, and the other 38 cases which received usual retral correction were investigated in control group. The coronal Cobb angle of the major curves , apical vertebral translation , body height , operating time and the amount of blood loss between both groups were evaluated pre - opera- tively, post -operatively and at final follow up. [ Results ] The mean follow - up period was 47months. The mean correction rate were 62. 50% , 42. 10% and 7cm in the study group compared with 42. 1% , 36. 20% and 3era in the control group for cor- nal Cobb angle of the major curves and apical vertebral translation and body height, significantly statistical difference was found between the two groups ( P 〈 0.05 ) . However, there was no significantly statistical difference in operating time and the amount of blood loss between the two groups (P 〉 0. 05) . [ Conclusion ] The innovation technology is safe for correction of the severe and rigid idiopathic scoliosis and has increased the clinical resuhs.
