摘 要:目的观察分析3.0T MRI对比增强全心冠状动脉成像(CE-CMRA)联合经食管超声心动图(TTE)在诊断患者冠状动脉狭窄中的临床价值,为进一步的治疗和护理提供诊断依据。方法选取自2010年9月~2012年4月本院心内科确诊或疑似冠心病并拟行冠脉造影(CAG)检查的102例患者,于检查前24h内行3.0T MRI对比增强全心冠状动脉成像和超声心动图检查,并于冠脉造影检查后对两种方法的检查效果进行分析评价。结果①TTE的阳性预测值和准确率要明显低于CE-CMRA(P<0.05);②两法综合出阴性预测值外,灵敏度、特异度、阳性预测值、准确率均明显增高(P<0.05)。结论 CE-CMRA和TTE联合诊断冠状动脉狭窄时诊断效果较好、准确率高,且无创伤、应用范围更广,可作为无法行CAG时的替代检查方法。Objective To obserVe and analyze clinical value of 3 0T MRI ontrast enhanced coronary magnetic resonance angiography (CE-CMRA) and transthoracic echocardiography (TTE) in the diagnostic patients with coronary artery ste- nosis, to provide diagnostic basis for further treatment and care. Methods We chose patients from September 2010 toin April 2012 who were confirmed or suspected coronary artery disease, before coronary angiography (CAG) examination, u sing 3.0 T CE-CMRA and TTE, then checked the effect of the two methods. Results (1) TTE positive predictive value and accuracy were significantly lower than CE-CMRA ( P 〈0.05) ; (2) Combined two methods, except the negative pre- dictive value, the sensitivity, specificity, positive predictive value, accurate rate were significantly higher (P 〈0.05). Conclusion CE-CMRA and TTE are better in diagnosis of coronary artery stenosis, with high accuracy, and no wounds wider range of applications, it can be used as a substitute check can not line CAG method.
