不同中医治法对慢性乙型肝炎病毒感染患者外周血IL-2和IFN-γ的影响

摘　　要：目的观察健脾法、补肾法、解毒法分别联合西药治疗对慢性乙型肝炎病毒(HBV)感染患者外周血细胞因子白介素-2(IL-2)和干扰素-γ(IFN-γ)的影响。方法将120例慢性HBV感染患者随机分为对照组、健脾组、补肾组和解毒组各30例。对照组给予拉米夫定治疗,健脾组给予拉米夫定+四君子汤治疗,补肾组给予拉米夫定+六味地黄丸治疗,解毒组给予拉米夫定+甘露消毒丹治疗。疗程1年。观察各组治疗前和疗程结束后血清IL-2和IFN-γ水平。结果治疗前各组血清IL-2和IFN-γ水平差异无统计学意义(P>0.05);治疗后各组患者血清IL-2、IFN-γ水平均显著上升(P<0.05),其中健脾组、补肾组上升较对照组显著(P<0.05,P<0.01),补肾组上升较健脾组显著(P<0.05),解毒组与健脾组比较差异无统计学意义(P>0.05)。结论补肾法改善慢性HBV感染患者细胞因子IL-2和IFN-γ失衡状态优于健脾法和解毒法。Objective To observe the effects of Traditional Chinese Medical remedy of strengthening the spleenr, tonifying the kidney and detoxification combined with Western Medicine on peripheral blood cytokines IL-2 and IFN-γ in patients with chronic HBV infection. Methods 120 patients with chronic HBV infection were randomly divided into control group, strengthening the spleen group, tonifying the kidney group and detoxification group, with 30 patients in each group. The control group was treated with lamivudine. The strengthening the spleen group was given lamivudine and Sijunzi decoction. The tonifying the kidney group was given lamivudine and Liuwei Dihuang pills. The detoxification group was given lamivudine and Ganlu Xiaodu pills. The treatment course was 1 year. The levels of serum IL-2 and IFN-γ were measured before and after treatments. Results Before treatments, the differences of serum cytokine levels among 4 groups were not statistically significant （P〉0.05）. After treatment, the levels of serum IL-2 and IFN-γ in- creased significantly in 4 groups （P〈0.05）. Compared with control group, the levels of serum IL-2 and IFN-~/ in strengthening the spleen group and tonifying the kidney group increased significantly after treatments （P〈0.05,P〈0.01）, and the changes were most obvious in tonifying the kidney group （with statistic differences）（P〈0.05）. There was no significant difference between detoxification group and strengthening the spleen group after treatments （P〉0.05）. Conclusion The method of tonifying the kidney is better than strengthening the spleen and detoxification methods to improve the im- balance of cytokine IL-2 and IFN-γ in patients with chronic HBV infection.
