骨关节炎患者血清及关节滑液中细胞因子的表达水平研究

摘　　要：目的探讨骨关节炎(OA)患者血清与关节滑液中细胞因子的水平及相关性。方法 2010年1月—2012年1月选择齐齐哈尔医学院附属第三医院87例OA患者作为病例组,选择同期体检健康者70例作为对照组,采用ELISA法检测血清和关节滑液中肿瘤坏死因子α(TNF-α)、白介素1(IL-1)和白介素6(IL-6)的水平,比较两组血清和关节滑液中各细胞因子水平的差异及相关性。结果病例组患者血清和关节滑液TNF-α〔(86±17)ng/L和(228±36)ng/L〕、IL-1〔(122±33)ng/L和(537±77)ng/L〕和IL-6〔(152±26)ng/L和(434±98)ng/L〕水平高于对照组〔(36±11)ng/L和(137±36)ng/L,(46±12)ng/L和(153±28)ng/L,(60±10)ng/L和(176±48)ng/L〕,差异均有统计学意义(P<0.05)。病例组患者血清与关节滑液中各细胞因子水平间均呈正相关(TNF-α:r=0.868,P<0.05;IL-1:r=0.920,P<0.05;IL-6:r=0.770,P<0.05)。结论 OA患者血清和关节滑液中细胞因子TNF-α、IL-1和IL-6表达异常增高,OA的发病与细胞因子表达密切相关。Objective To investigate the expression and relationship of cytokines in serum and synovia in osteoarthritis （OA） patients. Methods 87 OA patients admitted to the Third Affiliated Hospital of Qiqihar Medical College from January 2010 to January 2012 were selected as case group, and 70 healthy people underwent physical examination in the same period were selected as control group. Elisa method was used to detect TNF - α, IL - 1 and IL - 6 in serum and synovial. The difference and relationship of cytokines in serum and synovia between the two groups were compared. Results In the case group, the levels of TNF - α, IL - 1 and IL - 6 in serum and synovial were [ （86 ± 17 ） ng/L and （ 228 ± 36） ng/L], [ （ 122 ±33 ） ng/L and （537± 77） ng/L] and [ （152 ± 26） ng/L and （434 ± 98） ng/L], significantly higher than those of the control group, which were [ （36±11） ng/Land （137±36） ng/L], [ （46±12） ng/Land （153±28） ng/L] and [ （60±10） ng/Land （176 ± 48） ng/L] . The cytokines in serum and synovia in the case group were positively correlated with each other （TNF -α： r = 0. 868, P 〈 0.05 ; IL - 1 ： r = 0. 920, P 〈 0.05 ; IL - 6 ： r = 0. 770, P 〈 0.05 ） . Conclusion The cytokines including TNF - α, IL - 1 and IL - 6 in serum and synovial in OA patients are increased abnormally, and the occurrence of OA is closely related to the expression of cytokines.
