半髋及全髋在高龄股骨颈骨折置换中的应用对比

摘　　要：目的探讨半髋及全髋髋关节置换术在高龄股骨颈骨折中的应用效果及安全性。方法选取2007年1月至2011年1月在我院接受治疗的96例高龄股骨颈骨折患者为本次研究对象。其中行全髋关节置换术51例(全髋组),行半髋关节置换术45例(半髋组),观察两组患者手术时间、术中出血量、住院时间、术后并发症情况、疼痛情况及髋关节功能恢复时间等相关指标,给予1年以上随访,观察髋关节恢复情况,并进行评定。结果全髋组手术时间、术中出血量及髋关节功能恢复时间与半髋组比较有明显差异(P<0.05);两组并发症及1年后髋关节功能恢复情况比较无显著差异(P>0.05),半髋组术后疼痛发生率明显高于全髋组(P<0.05)。结论对于高龄股骨颈骨折患者,全髋和半髋置换术均为有效的治疗方法,可改善患者生活质量,但全髋关节置换术是治疗高龄股骨颈头下型骨折的较好选择。Objective To investigate the efficacy and safety of semi-hip and total hip arthroplasty in femoral neck fraetures. Methods Totally 96 femoral neck fracture patients in our hospital from January 2007 to January 2011 were elected and studied. 51 eases of patients （total hip group） were perfoiTned with total hip arthroplasty and the other 45 eases （semi-hip group） were performed with semi-hip replacement. The operation time, blood loss, hospital stay, postoperative complications, pain and hip joint function recovery time and other related indicators were observed. All the patients were followed up more than one year. Results The operation time, blood loss, and function recovery time of the two groups showed an obvious differenee（P 〈 0.05 ） while the postoperative complications and function recovery of the two groups showed no significant difference（ P 〉 0.05 ）. And the semi-hip group of postoperative pain was significantly higher than that of the total hip group （ P 〈 0.05 ）. Conclusion For patients with femoral neck fractures both semi-hip replacement and total hip replacement are effective treatments to improve their quality of life, but total hip arthroplasty is a better choice for the treatment of elderly femoral neek fracture.
