髋关节置换术与骨折内固定术治疗股骨颈骨折的疗效分析

摘　　要：目的比较髋关节置换术与骨折内固定术治疗股骨颈骨折的临床疗效。方法回顾分析2009年6月至2010年12月我院收治的145例股骨颈骨折患者的临床资料,按照手术方法分为两组,髋关节置换组和内固定组。髋关节置换组72例患者行全髋关节置换术,内固定组73例行骨折内固定术,分别对治疗效果进行评定。结果髋关节置换组和内固定组手术时间和住院时间比较无显著差异(P>0.05)。与骨折内固定组比较,髋关节置换组手术出血量和下床活动时间明显将低,术后并发症发生率明显下降,髋关节功能明显改善(P<0.05)。结论全髋关节置换术治疗股骨颈骨折可显著改善患者患肢功能,提高患者生活质量。Objective To explore clinical effects of hip replacement and internal fixation for fracture of femoral neck. Methods The clinical data of 145 patients with fracture of femoral neck were retrospectively analyzed. All the patients were divided into hip replacement group （72 cases） and internal fixation （73 cases） according to their different surgical methods. The effects were analyzed between the two groups. Results The operative time and the time of hospital stay were of no statistical difference between hip replacement group and internal fixation group （ P 〉 0.05 ）. The blood loss, complication and the time of forepart movement in hip replacement group were significantly lower than that of the internal fixation group （ P 〈 0.05 ）. The joint function of hip replacement group was significantly improved compared with the internal fixation group （ P 〈 0.05 ）. Conclusion Hip replacement can improve joint function and enhance the quality of life for patients with fracture of femoral neck.
