摘 要:目的 探讨组织追踪法评价兔甲亢性心肌病不同左室构型左室收缩功能变化的价值。方法 新西兰纯种白兔30只,随机分为实验组(20只)和对照组(10只)。实验组腹腔注射左旋甲状腺素(45μg·kg﹣1·d﹣1×28d)建立甲亢动物模型,依据超声参数可将其分为向心性肥厚组(CH组)和离心性肥厚组(EH组),采用组织追踪法测量各组实验兔心脏二尖瓣环的收缩期位移峰值(Ds);M型超声心动图测量二尖瓣环收缩期下移距离(Dm);Teichholtz公式法计算左室射血分数(LVEF)、左室短轴缩短率(FS),并对各组指标进行比较分析。结果 实验组中20只实验兔于第2、3周各死亡1只,最终纳入研究的有18只,其中CH组7只,EH组11只。EH组二尖瓣环Ds、Dm、LVEF、FS均较对照组和CH组明显减低(P<0.01);CH组与对照组比较,二尖瓣环Ds、Dm为正常低值,LVEF、FS未见明显减低。结论 组织追踪法能准确评估兔甲亢性心肌病不同左室构型左室收缩功能的变化。Objective To evaluate left ventricular systolic function of different geometic patterns in rabbits with hyperthyroid cardiomyopathy by tissue tracking (TT). Methods Thirty New Zealand purebred rabbits were randomly divided into control group (n=10) and experimental group (n=20).Hyperthyroidism animal model was established by intraperitoneal injection of Levothyroxine (45 μg·kg^-1·d^-1×28d) in experimental group. According to ultrasonic parameters, the ventrieular geometries of experimental group were divided into concentric hypertrophy (CH group ) and eccentric hypertrophy (EH group). The systolic mitralannular peak displacement (Ds) was measured by by. The systolic mitralannular displacement (Dm) was measured by M-mode echocardiography. The parameters of LVEF and FS were calculated by Teichholtz's formula. The parameters in all groups were compared. Results In 2,3 weeks, 2 of 20 rabbits in experimental group died, and eventually 18 rabbits were included in the study, including 7 in CH group and 11 in EH group. Compared with control group and CH group, the Ds, Dm, LVEF and FS of mitralannular in Eh group decreased (P〈0.01). Compared with control group, the Ds and Dm of mitralannular decreased, while LVEF, FS did not significantly reduce. Conclusion TT can assess accurately left ventricular systolic function of different geometic patterns in rabbits with hyperthyroid cardiomyopathy.
