IL-18、IL-12、TNF-α在妊娠期肝内胆汁淤积症患者肝功能异常中的检测价值

摘　　要：目的探讨白细胞介素18(IL-18)、白细胞介素12(IL-12)、肿瘤坏死因子α(TNF-α)在妊娠期肝内胆汁淤积症(ICP)患者肝功能异常中的检测价值。方法选取在无锡市妇幼保健院住院的ICP患者96例,根据患者血清总胆汁酸(TBA)、转氨酶的不同升高程度分成A1、A2、A3组和B1、B2、B3组,采用酶联免疫吸附试验(ELISA)法检测各组患者血清IL-18、IL-12、TNF-α水平。结果在不同TBA水平血清IL-18、IL-12、TNF-α的表达水平对比中,A1、A2、A3组的IL-18和TNF-α表达之间差异有统计学意义,P<0.05;IL-18和TNF-α水平与TBA水平均呈正相关(r=0.69和0.65,P<0.05);IL-12表达之间差异无统计学意义,P>0.05。在不同转氨酶水平血清IL-18、IL-12、TNF-α的表达水平对比中,B1、B2、B3组的IL-18和TNF-α表达之间差异有统计学意义,P<0.05;血清IL-18和TNF-α水平与转氨酶水平均呈正相关(r=0.63和0.56,P<0.05);IL-12表达之间差异无统计学意义,P>0.05。结论血清IL-18和TNF-α表达水平可以用于评估ICP患者肝功能异常情况,对判断ICP患者预后有一定的价值。Objective To investigate the testing value of interleukin 18（IL-18）,interleukin 12（IL-12） and tumor necrosis factor alpha（TNF-α） in intrahepatic cholestasis of pregnancy（ICP） in patients with abnormal liver function.Methods 96 ICP patients were selected in the hospital,They were divided into A1、A2、A3 and B1、B2、B3 group according to the different elevated degree of the serum total bile acid（TBA） and transaminase.The expression levels of serum IL-18,IL-12 and TNF-α in all groups were detected by ELISA.Results Compare of different TBA level of serum IL-18,IL-12,TNF-αexpression level shows,A1,A2,A3 group＇s IL-18 and TNF-αexpression has statistically differences,P〈0.05.IL-18 and TNF-αlevel has positively related（r=0.69 and 0.65,P〈0.05） with TBA,IL-12 express has no statistically significant difference,P〈0.05.Compare of different transaminase level of serum IL-18,IL-12,TNF-αexpression level shows,B1,B2,B3 group＇s IL-18 and TNF-αexpression has statistically differences,P〈0.05.IL-18 and TNF-αlevel has positively related（r =0.63 and 0.56,P 0.05） with transaminase,IL-12 express has no statistically significant difference,P〈0.05.Conclusion Serum IL-18 and TNF-αexpression level can be used to evaluate ICP patients with abnormal liver function,has certain value to judge the prognosis of ICP patients.
